- Am I getting severe leg cramps from my high-carb cheats or from working out my legs while in a ketogenic state?
Dear Jimmy and The Doc,
I have been low-carb/keto for the past two years. A few months ago I started getting pretty severe leg cramps that would wake me up at night. I lift heavy weights 2-3 days a week and I noticed I would tend to get the leg cramps a few hours after leg workouts. To help with the leg cramps, every night before bed I mix an electrolyte drink of 5g of magnesium, 1/4 tsp of salt and 1/8 tsp of potassium. I also try to have a cup of homemade bone broth daily and add more salt to it as well. When I drink the electrolyte drink daily, my leg cramps do not occur.
Whenever I have any sort of high-carb cheat meal, within an hour or two I get severe leg cramping. I have been strict keto of 20g total carbs and moderate protein of between 60-90g since March 2017. I have tested my blood ketones and I am getting readings of 0.5-0.8 mmol. I cheated last Sunday on Easter chocolates and three hours later I had severe leg cramps yet again. I also just returned from a two-day hiking event and took my electrolyte supplement both days.
I am sore today but do you think the leg cramps is from the hiking or the high-carb load? I just find it suspicious and almost predictable that if I cheat, the leg cramps come on. Any insight is appreciated.

He initially thought that I might have a bladder infection. I had no symptoms of illness, so this shocked me to hear this from my doctor. Is there any connection between eating keto and having a false elevated white blood cell count? Have you ever seen this before?

3. Can you go into more details about why some Ketonians deal with a pounding irregular heart rate and corresponding sleep issues?

Back in Episode 37, you answered a question about heart rate and sleep. But the answer did not identify the problem nor did it give a solution. I am having the same irregular pounding of my heart and sleep loss since I started keto. Normally you guys do research and give an answer that explains the cause behind the issues asked about and then you share what to do about it. But it seems like you swept the problem under the rug and gave a ”there's something else going on" response. But I’m sure a lot of other people have this problem and it deserves a real answer. I live keto and won't go back to my previous diet, but I still want to solve the pounding irregular heart rate and sleep problems since going keto.

- Is there a way to quantify the amount of ketones being burned and used by the body? What percentage of ketones go to the various parts of the body?
Hey Jimmy and Adam,
Loved meeting you guys at the Metabolic Therapeutics conference in Tampa, Florida earlier this year. I wanted to ask a question about how to quantify the ketone bodies being produced at any given time. The problem with testing blood ketones is it only gives you a snapshot of the ketones being produced in the blood, but not necessarily what is being burned at any given moment. Over time once you become keto-adapted, beta-hydroxybutyrate levels decrease ostensibly because they are being used more efficiently. In other words, a 1.0 mmol blood ketone reading of something who has been eating keto for a couple of years might actually be identical to a keto newbie getting a reading of 6.0 mmol. So, how can we measure the amount of ketones being burned and their distribution to the various parts of the body, especially the brain? Is there any way to know what percentage of ketones go to the various functions in the body?

I wanted to update you on my appointment with the surgeon last Wednesday. He insisted I eat that diet they recommended to shrink my liver enough to have surgery in May. But I am sticking with the ketogenic approach that I know works for this. My blood sugar is down to 83 mg/dL and I’ve had no hunger and feel so much better. Thank you for such an inspiring podcast.

In research, a super reproducible way to increase a person's endotoxin levels is to make them drink 3/4 of a cup of heavy cream.
Drink some cream that's high in saturated fat and 1 hour later all the markers of endotoxin damage skyrocket.
In fact, ANY MEAL that is high in saturated fat can create this type of endotoxin impact.
Endotoxins come from your gut bacteria and are associated with inflammation, arthritis, atherogenesis, insulin resistance, weight gain and even depression. You want to do what you can to protect yourself from endotoxins entering your body.
So should you avoid saturated fats like the devil? Are all those Paleo people wrong?
No.

1. Should I be concerned with a higher level of Firmicutes in my gut health which supposedly puts me at a greater risk for obesity?

I’ve been a listener since the first episode and enjoy listening to your podcast. I’ve gained so many valuable insights and appreciate the immense support you bring us as community of “Ketonians.” I have a question that I don’t think you’ve covered yet regarding intestinal microbiome and keto. I recently had my gut bacteria analyzed by Ubiome that indicated a major skew in the direction of more Firmicutes compared with Bacteroidetes at a 5.8:1 ratio. According to their commentary on my results, this makes more more susceptible to obesity.
So my question is this: Should I be concerned about this seemingly poor ratio of Firmicutes to Bacteroidetes that has happened since I started eating ketogenic? Or is this association of a preponderance of Firmicutes with a higher risk of obesity only hold up in the context of the Standard American Diet where we know eating too much fat in the presence of high amounts of refined carbs frequently result in obesity? Thanks for your help with this question.

2. How can eat keto when I have Variegate porphyria requiring me to eat more carbohydrates in my diet?
Hi Jimmy,
I love your Keto Talk podcast so much it overrides all other podcasts for me. In 2014, I made a commitment to get healthy at the age of 64 after a career in IT with a poor diet, lifestyle, and genetic issues that put me in a very unhealthy state. Eating a Paleo/primal diet, I am now down 30 pounds from my highest weight and all of my key blood markers are good with the exception of LDL and total cholesterol. But I’m afraid I can’t eat keto because I have hemochromatosis (high iron) and Variegate porphyria. Eating primal has significantly improved my iron overload, liver enzymes, and the time between needing to do a phlebotomy. While my iron overloaded liver seems to love low-carb, high-fat, my porphyria doesn't. I visited the Mayo Clinic to confirm the porphyria diagnosis I was told to eat 400g carbs daily. With the primal lifestyle I usually eat around 150-200 grams carb per day. However when I try to go lower than that the porphyria kicks in and I get increased pain and complications in the muscle and nerve tissues. It seems porphyria needs carbs which runs counter to my desire to be ketogenic. Is there a way for me to eat keto while also eating to feed the porphyria? Thank you for your help.

3. If you spread the carbs out throughout the day, does that create a lower insulin release?
Hi Jimmy and Doc Muscles,
Do people in general have less of an insulin release if carbs are spread out over the course of the day then eating them all at once? When I use to eat a normal carb diet, the theory was to eat smaller portions as a means of keeping insulin levels low. Would something like this help for someone who is very insulin resistant? I'm fairly insulin sensitive, so I use keto to stay healthy. I've been eating keto for about four months and enjoy doing n=1 experiments to see how my body reacts to any changes I make. I would love to hear your thoughts on this. Thank for all the info you guys provide to us and I always look forward to your new episodes. I wish you guys would create a forum for your large Ketonian community to talk to each other and find out what topics are hot in the world of keto.

- Do I need to eat a high-carb diet prior to visiting a very high elevation for a few weeks to utilize oxygen better?

Thank you for such an informative and funny podcast. It's such a great companion for me on my long drives. I have a question on doing keto while at high altitude. I live close to sea level and will be going on a business trip to a place at 17,000 feet above sea level soon for a total of 2 1/2 weeks. The client has advised us that we need to eat a high carb diet (75% carbs) in order to use oxygen efficiently at that elevation. My understanding is that when we are in ketosis we don't need to be as concerned with absorption of oxygen as we don't need it for the fermentation process to produce energy from glucose. Is this correct? If not, should I be eating high-carb in preparation for this this? Also, do you have advice on how to optimize energy levels in this situation?

I have Hashimoto's, so I am keen to see if keto can help with controlling my thyroid and hormones. I am 32 years old, 5’7”, and weigh 168 pounds. I lost 22 pounds on the hCG diet, but need a more sustainable way of eating to keep the weight off. My question for you is about this insatiable hunger I felt during Phase 2 of hCG that has continued with keto. I thought it would subside with all the fat I’m eating, but it has not. All I can think about is finding more food, especially meat and fat. This food obsession is unrelenting and I’m at a loss about what to do. I’m continuing to gain weight and I’m frustrated by this.
My supplement intake includes Magnesium, Selenium (to help with Estrogen), activated B complex, Adrenotone (Adrenal herbal supplement to assist with Adrenals), high dosage Vitamin C (ph balanced variety), probiotics and a colloidal mineral complex. I also take Desiccated Thyroid Extract 45mg and naturally derived progesterone cream (applied from day 12 of cycle to menses). I drink plenty of water (about 3-4 liters per day), eat plenty of salt, and consume lots of raw organic grass fed butter and organic coconut oil. I steer clear of dairy due to the rapid weight gain I get from it with the exception of butter. My urine ketones are on the low end, so I realize that’s probably part of the reason for my struggle.
I understand this insatiable hunger is somehow hormone related, but I am wondering if you are able to explain further what might be going on and what physiological processes are going on to cause this. Thank you so much for what you do! I am in awe of your wealth of knowledge and appreciate what you are doing for all us Ketonians out there.

You guys are transforming lives with your podcast and you’ve made me feel like a keto evangelist when the topic comes up in my circles. You are to carbaholics what AA is to alcoholics. I have Fibromyalgia/chronic fatigue syndrome and using the ketogenic diet over the past 4 months has enabled me to improve these conditions greatly.
I got Fibromyalgia/chronic fatigue while doing daily cycling training rides for up to 3 hours after work while being deficient in Testosterone, Growth Hormone, and Thyroid issues as a result of having Empty Sella Syndrome. The stress of that combination has set me up for this illness. Sadly my diet was loaded with carbage for decades and thankfully I’m now 100% ketogenic for life at the age of 49. My hope is keto will help me recover along with appropriate hormone replacement. Is this possible?

2. Will consuming 30g carbs or less with IF cause me to halt or rev up my weight loss efforts?

I’m a healthy 34-year old Ketonian and need your help to clear up some confusion I’ve been hearing about keto and intermittent fasting. A nurse practitioner at a low-carb clinic told me that doing IF and going below 30g carbs daily will halt your weight loss. Another Ketonian says daily IF with 20g carbs daily for at least 30 days will help jumpstart my weight loss again. I’m currently keto-adapted and have been for quite a long time. 3. Should I be concerned that my ketone levels are so high early on in my ketogenic journey?
Hi Jimmy and Doc!
Loving your podcasts and listen on my daily walks! I am new to keto and learning everything I can about it soaking it in like a sponge. My question is regarding ketone levels. On Day 5 of starting my ketogenic diet, my ketones were high in the afternoon at around 8 using the Ketostix. I made sure I was plenty hydrated and my urine was clear. I tested again a couple of days later in the morning right after waking up and my ketones were also high. I Googled and searched all over to find any information about why I am experiencing such a high reading like this. Do I need to eat more carbs than the 10-12g daily I am currently eating? I feel fantastic and have tons of energy after having only a couple of days of the keto flu. I consume around 1400 calories a day and stick to 60g protein and 100g fat. I’m loving the food I get to eat now, but these higher ketone levels concern me. Should I be concerned?

I have been enjoying eating keto as a 72-year old woman and I have been blessed with excellent health and no medications. Despite being overweight, I have perfect health markers. I probably need to lose 50 pounds and I’m working on finding a local physician to help guide me on my ketogenic diet. One of the employees at this one clinic told me a woman my age should never be in ketosis. Am I too old to be ketogenic?
