1. Do I need to detox my liver and gallbladder before attempting to go keto?

Hello Jimmy and The Doc!

Ever since I decided to adopt a ketogenic lifestyle, I’ve wondered if I should do a number of liver and gallbladder flushes before trying to get keto-adapted? It’s usually recommended to eat low-fat for a few days during this flushing period so that would likely kick me out of ketosis anyway. I feel like my body has a difficult time detoxing and I wanted to do a series of liver flushes to get my liver working optimally again.

I am a Holistic Lifestyle Coach and have the ability to run labs for liver function. My mother, grandmother, and great grandmother have all had to have their gallbladders removed and I want to keep mine! Would really love to hear your thoughts on this.

2. Is using exogenous ketones an effective way to boost ketones when they are at their lowest?

Hey guys,

Is there any emerging research on what the most effective dose of exogenous sources of beta-hydroxybutyrate should be? And what is the best time of day to use them? Through my own N=1 experimentation, I have found that I'm out of ketosis for about 3 hours after I wake up in the morning. Then by midday I'm right back into ketosis with levels steadily increasing as the day goes on. After working out in the evenings, I am usually in the red zone on the Ketonix.

But then by the following morning I'm back to green again. So I'm wondering if I should take a supplement like KETO//OS in the mornings upon waking up since this is when my levels are naturally at their lowest or if I should take it in the afternoon or evenings to accentuate my natural rhythm. Are there any studies looking at the relationship between circadian rhythms and ketones or the use of exogenous ketones that could help me in my situation? My purpose in eating ketogenic is disease prevention and anti-aging.

3. Is my depression preventing me from getting the full benefits of keto? Do depression drugs work differently in the presence of ketosis?

Greetings gentlemen,

I’m sure many people who are trying the ketogenic diet suffer from major depressive disorder. I recently stayed in nutritional ketosis averaging 1.0-5.0 mmol/L and heavily supplemented with electrolytes for 5 weeks but did not experience the increased energy that I hear being reported by others. Although I lost weight and didn’t have to eat as often, I still had some pretty intense carb cravings.

Could my depression be interfering with the benefits of nutritional ketosis? And if I push through these cravings now, will remaining in a ketogenic state eventually help me deal with the depression? I also wonder what some of the most commonly-prescribed selective serotonin reuptake inhibitors such as Lexapro, Prozac, and Zoloft do in conjunction with keto to interact and work differently than a standard diet.

4. Can I use green banana flour resistant starch and stay in ketosis while taking care of my microbiome?

Hey Jimmy and Adam,

I'm starting on a new health mission to feed and nurture my microbiome. The mechanism for this feeding is a daily high-fat smoothie that contains coconut milk, protein powder, MCT oil, some berries, and banana flour resistant starch to make a high-fat, moderate protein, low-carb keto smoothie. I have a few questions for you guys about this:

- Is this Green Banana Flour Prebiotic Resistant Starch keto-friendly? I've read on the Internet that the starch does not spike blood sugar. Is that true? The product I'm using has 7.5 grams of carbs per tablespoon. What say you?

- Will resistant starch on a ketogenic diet over time transform my microbiome from being fat to lean?

- Do you have additional suggestions or thoughts for the care and feeding of the microbiome while remaining in a state of nutritional ketosis?

5. Is taking L-Glutamine a good or bad idea when you are on a ketogenic diet?

Thank you for your podcast and I never miss an episode of Keto Talk. I have been eating LCHF for 4 months and have lost 40 pounds so far. I have become interested in taking it to the next level with ketogenic and I am having a hard time getting into ketosis while my weight loss has stalled out. I am taking 8 grams of L-Glutamine a day on the recommendation of Maria Emmerich from one of her books and it definitely seems to help me with sugar cravings. But I heard that L-Glutamine can turn into sugar through gluconeogenesis. Now I'm totally confused and need your help to clear up whether I should be taking this supplement or not.

6. Why is my blood pressure still so high despite cutting my carb intake?

Hi Jimmy and The Doc,

I enjoy your show and like hearing about all your experiences and advice concerning keto living. It seems that most people are able to lower their high blood pressure readings very quickly after cutting their carbs. But this has not occurred in my case, even after a year or so. I did hear that there could be other reasons apart from eating high-carb that could lead to high blood pressure. What are they?

7. Should I be concerned about toxins from filter fat in my bacon grease fat?

G'day Jimmy and Adam,

A huge THANK YOU for your helpful and informative podcast and for being so generous with your experiences and knowledge beyond this podcast! You both are changing lives for the better!

My question relates to cooking for a ketogenic lifestyle. I hear many people suggesting cooking with bacon grease or bacon fat as we call it here in Australia. I was talking to my local butcher about and mentioned that I cook with bacon fat and he became very alarmed by this. He explained that there are two types of animal fat—one healthy that is what he describes as clean fat and one not so healthy which he calls filter fat. The filter fat contains massive amounts of toxins which is why my butcher recommends cooking with lard or tallow sourced from fat around the kidneys.

Is this something Ketonians should take into consideration or not? Personally I tell people to eat the best your budget will allow and this isn't always organic and grass-fed. We eat bacon everyday and I'm certainly not changing that habit. But maybe the idea of reusing the bacon fat saved from cooking our morning bacon isn't such a good idea after all. I just thought I would bring this up as it is relevant to those of us in the keto community. What are your thoughts?

8. Why is peanut butter demonized and almond butter glorified on a ketogenic diet?

Hi Jimmy and The Doc,

I’d like for you guys to settle the seemingly neverending debate about why peanut butter is technically not keto,and why almond butter is so much better. The carb difference between the two is negligible assuming you’re getting a nut butter that has no sugar added to it and the fatty acid profile doesn’t look dramatically different either. I do notice that almond butter has a SLIGHT advantage over peanut butter in the omega 3 to omega 6 ratios but both are very low on omega 3’s as compared to their respective omega 6 ratios. And both look to be within .5 grams of polyunsaturated fat per 1 oz. serving.
So why does peanut butter get demonized while almond butter looks to be a very similar food from a nutritional perspective? I thought this would be a fun topic for you guys to cover on Keto Talk as I know this has been a hot topic of debate on the Internet within low-carb and ketogenic circles.

9. Does being in a ketogenic state produce complications with taking birth control pills?

Dear Jimmy and The Doc,

Thank you for your outstanding podcast because it adds so much value to my life. I have a question that I hope you can help me with. My girlfriend has joined me in eating a well-formulated ketogenic diet for just over a month and she feels fantastic in almost every way. The only problem is her period has arrived sooner than it usually does and lasts longer, too. When it happened once, she thought it was an anomaly but then it happened again. She is taking birth control (Lutera) and always had regular and consistent periods while on the pill with little to no side effects.

Are there any potential complications with taking birth control while on a ketogenic diet? If the birth control is causing this issue, what alternatives does Doc Nally recommend? Or will all of this come back into balance with time?

10. Are there any studies looking at ulcerative colitis and a ketogenic diet?

Hello Jimmy and Doc Nally!

Love the show, especially when you two tackle the latest research, and I have been a faithful listener and Ketonian since I discovered your show back in March!

I have a family member who is suffering from ulcerative colitis and I was wondering if you guys might be able to shed some light on this disease and how a ketogenic diet might improve or reverse it. Is there any research on using a nutritional therapy like keto to treat it or are people doomed to be on medications forever?

11. Does a high-fat diet like ketogenic increase inflammation in the prostate?

I have been having some serious prostate problems and they seem to be getting worse on the keto diet which I've done for about 6 months. My blood work is great though. I ran across a study from the American Urology Association that states that high-fat is inflammatory for the prostate: see page 3 of 4 under Epidemiology

I am losing my mind over this prostatitis. It’s very painful even though the doctors all say they cannot find anything. Maybe I am the one in a million exception to the rule where eating high-fat does not work for me. Anyways, any feedback you might have would be greatly appreciated.

12. Is there any issue with someone taking a fat-burning supplement while eating ketogenic?

Hi guys,

I've listened to the majority of your episodes and I think I have a topic you haven’t addressed yet. What are your thoughts about the plethora of stimulant-based fat-burning supplements that are being marketed out there? I've heard Dr. Atkins scolded his patients very earnestly in his practice for "cheating" instead of just believing in the diet. You can never really be sure that these supplements actually contain the intended ingredients, but the fact is these stimulants typically are intended to stimulate catabolic hormone pathways supposedly to enhance fat burning and "promote thermogenesis" (in the calorie counting paradigm). But if you're insulin resistant in some way, isn't it probably a really bad idea to be spiking catabolic hormones all willy-nilly when part-and-parcel of insulin resistance is an impaired ability for insulin to control these hormones at the expense of letting your body find the right levels through the ketogenic diet itself?

13. Does an increase in Vitamin D levels diminish ketone levels in the body?

Hello Jimmy and Dr. Adam,

I have a simple question for you: Does Vitamin D impact ketones? I only ask this because I have noticed a few times some noticeably lower ketone readings after a long day in the sun. I have a feeling movement while outside is also playing a role in this. Any thoughts on this?

14. Is a ketogenic diet an acceptable nutritional therapy for people with organ transplants?

Hello Jimmy and The Doc,

I’m wondering about the impact of a ketogenic diet for organ transplant recipients. I had a liver and kidney transplant in 2001 and was starting to experience problems after about seven years when my husband and I switched to a low-carb diet and very quickly saw all my markers improve. We’ve been eating this way since 2009 and we’re both in great shape. I’ve been telling my nephrologist and my hepatologist what I’m doing, but I can’t persuade them to recommend this way of eating to other transplant patients. Am I just lucky to see my health improve eating keto or could this help other transplant recipients, too?

15. What do I need to do to experience the robust energy that is supposed to come from ketosis?

I’ve been more or less keto for over two years now and I have to say I don’t really have a “feeling” of when I’m in ketosis. My energy level never really went up all that much and I hear people talk about how great they feel in ketosis. The truth is I’m envious of them because I’ve never experienced this myself. I know there are a LOT of things that relate to energy level: stress, emotional issues that can drain a person, thyroid, electrolytes, etc. How can someone like me who is keto obtain the typical energy level that other Ketonians are experiencing? Where would you start and what would you want to rule in or out? I have a very low CRP, no calcium in the heart, TSH normal and more.

16. Is eating a high-fat diet without significantly reducing carbohydrates a concern for cardiovascular health?

Hi Jimmy and Doc Muscles!

I’ve been listening to Keto Talk since the beginning and really enjoy the banter between you two! You both do a great job of making all the information you provide easy to understand. I have a question that seems to have been answered in a roundabout way previously on your podcast. But I would love an answer that is specific to my concern.

I have been dealing with autoimmune issues for a few years now. Psoriasis since my 20s, and possible rheumatoid arthritis for about 2 years now. I’m 40 years old and had always followed recommended dietary guidelines, but that didn’t prevent these issues from happening. I follow a mostly AIP, lower-carb diet, but I know that I am not in ketosis most of the time.

My carbs include small amounts of fruit (usually about 1 serving per day), sweet potatoes, and all vegetables. I sometimes make AIP friendly treats that are low in sugar, but do contain some starch. I now eat higher amounts of fat than I ever have from Kerrygold butter, cream, avocados, coconut oil and olive oil. I feel really good most of the time and love how I can wait longer between meals and don’t seem to get hangry. I stay quite active, work standing up, kick box 2-3 days per week, do a little yoga, and still mix in a little running, biking and swimming from my triathlon days.

My question for you guys is this: If I am not in ketosis most of the time while eating 50-100 grams of carbs with my higher consumption of fat, am I jeopardizing my cardiovascular health? I don’t believe I have a blood sugar issue. I test it sporadically, and even after a higher carb meal, it is under 120. My total cholesterol jumped from 208 to 269 during the last 3.5 years, LDL went up to 139, triglycerides in the 50s, HDL in the 90’s. Of course, my HMO doctor wants me to retest in three months because it is so high.

17. Can toxins released from fat stores while eating keto lead to eczema?

I’ve been doing keto really well and keeping my daily carbs under 20 grams and I feel great. My energy and mental clarity are awesome and my waist has dropped from a size 41 to a size 35 in only 6 months. But I've developed a serious and painful case of eczema on my feet. These blisters are huge and my guess is that my body was holding onto toxins in my fat cells which are now being released and overloading my body leading to the eczema. Am I right about what’s happening or is there some other mechanism at work with this? If it is toxins being dumped into my bloodstream, then what is the best method to deal with this? Should I slow down on my ketogenic diet or should I attempt chelation or some other method to bind and remove these toxins.

18. Is it better to have a little bit of real sugar sources instead of sugar alternatives?

Dear Jimmy and Adam,

For long-term health, I eat low-carb/higher fat and I prefer to eat whole foods. So when I see recipes that use erythritol, stevia, and similar sugar alternatives, I replace those with honey, pure maple syrup, molasses, table sugar, and brown sugar instead. I also cut the amount of sugar in recipes by about 50-70% or sometimes omit the sweetener entirely.

Erithrytol and other "tols" also upset my stomach and gut. What so you think about using small amounts of "real" sugars as I described? I am fortunate to have a healthy metabolism and can tolerate a few more carbs than my husband, for example, who is carb-sensitive like you guys. Would a small amount of a sugar (say 2 Tbs total at the most) be too much for him?

1. Should I ignore my body fat percentage if all my health markers are superb while eating keto?

Hey guys, I love Keto Talk and hope you can help me with my question. Here it is: How important is my body fat percentage number?

Both my parents died in their 70s of heart disease and dementia and I was slowly following the same path. One year ago I decided that I would do whatever it takes to not be like them and discovered keto and intermittent fasting. My brain and skin are now clearer, I’ve lost 10 kg and for the first time in my life at the age of 44 I have great cholesterol, blood pressure, heart rate, liver, glucose and vitamin D levels. My psoriasis and inflammation are gone, I sleep better and have more energy.

But my body fat percentage measured by a Tanita scale has plateaued at around 32%. Last year when I began eating keto, it was around 38%. My goal is to be in the "normal range" but if everything else is great, should I just ignore the body fat percentage?

2. Does eating ketogenic reduce your desire for consuming alcoholic beverages?

Thanks for your podcast and the Clarity books.  I'm 8 days into eating 20 grams of carbs and moderate protein and feeling great. Interestingly, my habitual hankering for a glass of wine or vodka soda at 5PM has totally vanished. I haven't heard about this effect anywhere.  Have you guys heard of this?

3. Can taking blood pressure-lowering medications negatively impact ketone levels?

Do blood pressure medications, such as thiazides as diuretics, impact my ketogenic diet in any way? I know that these drugs have a tendency to spike blood sugars and assume that would mean they would be detrimental to getting into ketosis.

4. Should you cut your meals in half to minimize the negative impact of protein on ketosis?

I was wondering if cutting meals in half and spacing them out by a couple hours is good for keeping the protein lower in one sitting. I make an omelet with 27g of protein, 55g fat, and 1g carb and I eat half and then save the other half for three hours later because I'm not sure whether or not that's too much protein at one time.

Any advice on this would be awesome!

I’ve read Keto Clarity and many other books about ketosis, but I’ve still got questions about protein.  Here’s the deal.  For my height of 5’6” and age of 65, I should be getting in around 80 grams a day.  But what happens when I’m not hungry, which is not unusual when you are keto-adapted, and I only end up eating one meal in a day. Am I supposed to eat all 80 grams in that one meal?  That’s a lotta protein as well as a lot of food to try and choke down.

5. Does eating keto make your blood pressure drop too low and what can you do about it?

Hi Jimmy and Adam,

I'm eating ketogenic and have been experiencing what I thought was brain fog which turns out to be low blood pressure.  A friend recommended I measure my BP and it was 106/66 yesterday. I stopped at a pharmacy on the drive in to work this morning (the feeling is most noticeable when I am driving) and measured three more times with readings of 114/72, 108/66, and 107/63.  

I tried supplementing with salt tablets to see if that would help, but this didn't make me feel any better and my hands and feet started swelling. I've always had perfect blood pressure before starting keto. Other than adding more salt to my diet, are there any other ways to increase my blood pressure to a more normal range?  I don't want to abandon this way of eating, but if carbs are going to make me feel better I may have to.

6. Why are my triglycerides high while eating a ketogenic diet?

Hi Jimmy and Adam! I've really been enjoying your Keto Talk show! I've been doing a zero-carb keto for four months. I'm feeling great and really enjoying it. However, my triglycerides are high at 180. My other parameters are all good. Fasting blood glucose is 60, morning blood ketones are between 3.0-5.0 mmol, HDL is over 50, blood pressure is 98/73. Are my trigs high because I'm underweight and have been losing weight? My body fat is just 14.9% and my BMI is 17.

Can you guys talk about saturated fat just not agreeing with me? I noticed a high quality sugar-free chocolate bar with 22 grams of saturated fat would send my heart racing for 1-3 hours. Now I am leery of consuming anything with a high saturated fat content.

I have mindfully used monounsaturated fat oils like avocado and macadamia nut oil and really noticed this disappear completely. I get the rest of my fat from avocados, macadamias and pecans, and heavy cream as I tolerate dairy very well. I had a comprehensive food allergy testing done and nothing came back with a sensitivity. When I attempted to have a tablespoon chunk of cream cheese on my avocado, the same thing emerged with a racing heart for a couple of hours. The same thing happened with a couple of tablespoons of grass-fed butter. This seems to show up within minutes of ingesting saturated fat.

Keto has changed my life, giving me outstanding energy and stamina, much improved sleep, and a mental clarity never experienced before. Am I just stuck only eating monounsaturated fats on my ketogenic diet?

You guys are beyond awesome! Thank you both for your all the wisdom you share weekly.

8. What are the exact signs I am looking for that show I’m fully keto-adapted?

I’m a nutritionist who was a vegetarian for years and most recently Paleo and now here I am doing keto.  I have a blood ketone meter and have ketone readings consistently between 1-3. I’ve been at it for a couple of weeks now and in excellent health. What I want to know is what the exact signs are that am I looking for in my body or blood ketone levels to make sure I am fully keto-adapted?

9. Why is my weight loss so slow while eating keto?

Hi Jimmy and Doc Muscles, 

I am a new listener to your wonderful podcast and love it. I look forward to every episode.

I’m a lifelong dieter losing and gaining the same 70 pounds or so many times. I love the keto diet and believe in it since beginning it in February. I have only lost 18 pounds while my husband has lost 65. I know I shouldn't compare myself to him, but I wonder if the reason I am going pretty slow on this is that my body is saying "yeah, right. Here we go again."

I keep track of my macros as I keep a food diary. This so-called uncomplicated way of eating has become a very complicated way of eating for me.  I am staying under 20g of carbs daily. I do enjoy a daily diet soda and I have a fairly sedentary lifestyle. I will admit I don't think I have been as faithful as I should be from time to time as unfortunately my carb cravings really haven't subsided. I find myself hungry often. 

I am 56 years old and post-menopausal. I use urine ketone test strips and I have NEVER registered as anything other than "trace." Is it possible I not only have never obtained ketosis, let alone become keto-adapted? Any advice?

10. Is percentage of carbs or the number of carbs most important to get into ketosis?

Hey guys,

I love your show for all the good advice and knowledge you share. My question is about carb intake. I am currently struggling with the amount of carbs to take in. Currently I'm consuming about 50-70 grams of total carbs a day and if you subtract fiber it would be more like 30-50 grams of carbs a day.

I'm active lifting 5 times a week and run or bike another 3 times and am wondering if carb percentage is more important (I'm currently at 10% of calories in my diet) or the 50-gram mark?  Any suggestions about this? Am I doing this all wrong?

12. Are there some people who can’t tolerate eating a high-fat diet?

Hello Jimmy  & the Doc! I'm a huge fan! If podcasters had groupies, I would be that groupie with you guys. You've helped me tremendously, most recently, in convincing my husband that we needed to slow down on the peanut butter!

Here's my weird question I haven't encountered anyone else talking about this who didn't have their issues resolved by "giving it time" or by taking some kind of bile or enzyme supplement, so I'm a little worried.

My gallbladder was removed several years ago. When I first started eating keto, I had some loose stools that evened out more or less as I continued this lifestyle. Then it came back, and it was so much worse. I'm not sure if I can identify exactly when it returned, but it seems I'm not digesting anything anymore and my weight loss has stalled. The last 8 months or so have been very high stress, so I'm ashamed to admit I fell off the wagon a few times for a few weeks at a time. The liquid bowel movements would stop in those periods when I was back on carbs.

I take an ox bile supplement with most of my meals. I eat grass-fed gelatin on an almost-daily basis to work on repairing my gut. I eat plenty of greens and drink plenty of water. I've cut out most supplements to try to figure out if one of those was the culprit. I quit dairy for several weeks, no change.

I think my question is this: Are there some people who simply can't tolerate high-fat? If so, what is the lowest amount of fat that I could consume and still remain in ketosis? I realize that may be hard to answer and may vary from person to person...but I had to ask

I'm loving this new podcast and as a fellow Southerner, I also love love love your way with words. I have a few questions about an MCT oil-based ketogenic diet as advocated by people like Dr. Terry Wahls and Paul Jaminet that has added a layer of confusion to my understanding of a ketogenic diet.

It seems unbalanced to me to rely mainly on MCT oil and coconut oil as my primary sources of fat just so I can consume upwards of 80g carbohydrates in my diet as Dr. Wahls and Jaminet suggest. Do we really need to add in so many more vegetables and supplements so that we're not in danger of scurvy and other health calamities while eating keto? And they both push resistant starch which I know you’ve covered on your podcast previously, but can’t you just eat green leafy vegetables and kimchi to cover gut health?

Neither Dr. Wahls nor Jaminet seems to believe it's ideal to remain on a ketogenic diet over the long-term, and yet both of you seem to be thriving on it after many years of experience. Can you help clear up this major confusion I’m having about what to believe? Thanks so much for your great contributions and wonderful spirit.

1. What’s the purpose of fasting on a ketogenic diet?

Hey Jimmy and Adam,

I’m learning that on a standard American diet, cutting calories just doesn’t work. I think the reasons are because the metabolism slows accordingly, cravings kick in, and that leads to binge eating junk food or just more food when the willpower disappears. Why doesn’t this same logic apply to fasting on a keto diet? If cutting calories doesn’t work on the standard American diet, then why is fasting on a ketogenic diet beneficial?

2. Do you need to eat a calorie surplus while in ketosis to build muscle?

Hey guys,

I love your show and I listen in every week. I've often read that a calorie is not necessarily a calorie while researching keto and I believe I've heard you say the same thing on your show. My question is in regards to strength training, specifically powerlifting. I often see most of the fitness experts say you need to be in a caloric surplus to build muscle. Oftentimes these calculations work out to well over 3,000 calories. Eating a ketogenic diet, I don't know if I could ingest that much food without feeling stuffed all the time.

Is it possible that the old equation that says you need to be in a calorie surplus is not necessarily true in ketosis because you use a portion of your body fat to supply those calories? In the high-carb world your body would break down protein to supply the extra energy from glucose. But because we burn fat fuel, it makes me think perhaps it works differently for us Ketonians. I've been struggling to find a good answer to that question and I hope you and The Doc can help.

3. Should I add in a few more carbs to my keto diet, especially at night, to help with a pounding heart?

I love your show. It is chock full of great info and you two are both entertaining and fun to listen to!

My question for you is this: Does the liver require adrenalin to make ketones? I ask this question because my heart is sensitive to adrenalin and it has been keeping me awake at night with noticeable pounding at night. I've tried adding in more electrolytes before bedtime which hasn’t helped. I've just begun my sixth week of keto and I had my heart checked out earlier this year via echocardiogram-it is normal.

Could I still be in ketosis if I raised my carbs to say 40g daily as a means for dealing with this? If I do that would my fat intake remain the same, or should I decrease it? Maybe if I just eat a few more carbs before bedtime it would help? What do you suggest?

It's frustrating because I want to remain keto, but I want to sleep at night too.

Dear Jimmy and Dr. Adam,

I am a 41-year old woman with 2 children and have been very active all of my life, playing NCAA women's basketball for 4 years in college, and have since run two full marathons and at least 15 half marathons.  I still have at least 2-3 workouts a week as I chase after my kids, averaging about 14K - 20K steps a day on my Fitbit.

But I have been struggling with one major problem—a consistently elevated blood sugar. I have Hashimoto’s and while pregnant with my second child, I was diagnosed with gestational diabetes.  During that pregnancy I was extremely disciplined watching my carbs as directed by my doctor. And yet I still couldn't control my blood sugar and was required to take both slow acting insulin shots at night, and fast acting shots of insulin during the day.  I thought that this would improve after having my son, but it didn't.

Since the birth of my son in 2013, I have not been able to get my blood glucose under 100 mg/dL. I was on Metformin, but it didn't seem to help. In 2016 I went off of Metformin with my doctor's permission to see what would happen. Nothing has changed, so I was excited when I learned about the ketogenic diet. I started eating keto in mid-April 2016 and have been doing it ever since. I have lost 22 pounds (with 18 more to reach my goal) and I feel AWESOME with inches lost, increased energy, and improved focus. I’ve even done some extended fasts of 48-60 hours, but my blood sugar hasn't budged. My average morning fasting blood sugar level is around 105-115. I test my ketones with the Ketonix breath meter and it shows I’m on the low end of nutritional ketosis.

I intend to stick with the keto lifestyle as it is extremely easy for me and I love how I feel. I forgot to mention that my A1c has been around 5.2 for the last few years (which is encouraging), my cholesterol under 200, and my triglycerides around 130.  I guess that overall my numbers are good but I'm obsessed with that fasting glucose reading.  It seems to be my nemesis. 

I also forgot to mention that I have a Ph.D. in cell and molecular biology and every single word of science that you have described on your podcasts is spot on, and aligns with everything that I have been taught about cellular metabolism. Any words of wisdom on the timeline for lowering my fasting blood sugar level? Or, am I overly focused on this number? Is it worth going back on a low dose of Metformin again?

1. Can a ketogenic diet reduce plaque build-up in the arteries? How long will it take?

Most of us who are on the ketogenic diet tend to be overweight and have been for years. I would dare say many of us also have some plaque buildup in our arteries to some degree and are concerned about our heart health. So my question for you is this: what is the best way to reduce this plaque build-up and how long would it take to reduce it? I’ve heard you mention that your arteries are clean and in good health after many years of being on the low-carb/keto lifestyle.

2. Why am I getting winded and lacking energy climbing stairs since starting keto?

Hey guys,

I love your podcast. I've been devouring every episode like a fat juicy ribeye. I have been on my keto journey for about a month now and have lost about 17 pounds so far. I’ve noticed a lot of improvements in many aspects of my life from energy to eliminating my mood swings. But there is one thing that seems to boggle me—climbing stairs.

You would think that losing 17 pounds would make it easier to climb stairs, but it seems harder since I have been on this journey. I work on the second floor and have to climb three flights of steps everyday since there is no elevator available. I'm huffing and puffing by the end of it and my quads are burning, but only temporarily. I don't ever feel sore, but I almost always feel exhausted from this.

Are my muscles atrophying eating keto? Or since running on fat is like running on diesel, do I just lack the burst of energy that I had when I ran on sugar?

I’ve done moderate exercise during the week since I started the diet, including body weight squats about twice a week. I would imagine that my muscles are getting stronger but this stair climbing challenge seems odd.

Hello Jimmy and Adam,

First of all, great podcast!  Also, Jimmy I'm a big fan of all your bestselling books.

I recently converted to keto about 3 months ago consuming about 40 grams of carbs per day.  However, my urine pH has become low at 5.5 according to a 24-hour test prescribed by my urologist.  This test was prescribed because of some kidney stone issues which started several months before going on the ketogenic diet.

After reading up on low pH and how it can negatively affect my health, I think that I may be eating too many high acidic foods on my keto diet.  So many of the tasty keto foods I eat are very acidic.  I have started drinking high pH water and my urologist prescribed 1620 mg Potassium Citrite twice daily to elevate the pH.

Can you and the Doc please discuss the aspect of pH (in the body, blood, urine, saliva, etc.) and the keto diet on your podcast?

It is my understanding that dietary fat has no insulin response.  Let's assume I eat the my target level of carbs and protein, but I overeat with regards to target grams of fat.  How exactly would I store body fat from overeating dietary fat if it has no insulin response.  What metabolic mechanism would make fat from excess dietary fat?

3. Are the effects of gluconeogenesis from too much protein overstated?

Hey guys,

Just when I thought I understood why going over my personal protein limit would cause higher blood sugar, along comes this article: http://ketogains.com/2016/04/gluconeogenesis-wont-kick-you-out-ketosis/ …which states:
"The rate of conversion of non-glucose substrate to glucose (GNG) is relatively fixed – it is slow and consistent. The rate of GNG does not seem to be impacted by the protein content of the diet."

Now I’m confused. Is it excess protein consumption that triggers the negative effects of gluconeogenesis raising blood glucose levels and reducing blood ketones? Or is there something else going on with glucagon production or beta cell function? Could this be what causes an overnight rise in glucose?

Hi guys,

Love your show and I'm a long time listener of Keto Talk and The Livin’ La Vida Low-Carb Show. I've lost 30 pounds and have kept it off for a few years. I wasn't unhappy with my weight and just wanted to maintain it for a while. Now I am working on getting another 20 pounds off and I would be satisfied with it ending there. I would still be a little overweight but I'm okay with that.

Here's my problem: I’m a little frustrated by the fact that my weight will not budge during my period. In fact, I usually gain a few pounds and then it goes back to my normal weight with a few days after the period ends. But it does not go any lower no matter how low-carb I go during that week. Why does this happen? Is there anything I can do differently to make the weight come off during this time?

- Confusion about ketogenic diet continues to persist online

Hi Jimmy and The Doc,

Thank you for answering my question about undereating on keto in a recent episode. I wanted to follow up with you. Even though my question was quite vague in my personal details you still knocked it out of the park. Doc was right my protein was too low, my adrenals were stressed, and I had food allergies.

I've fixed up my diet and increased my protein and started taking supplements for adrenal support and wow what a difference. I know it will take some time as my body is still healing but I already feel so much better. I think I now know where it all went wrong. I started using an online keto calculator and it led me to make choices that weren’t right for me. I now test blood ketones as well as breath ketones with a Ketonix.

There is so much conflicting information about the keto diet out there I think people need to find a reliable source and stick to that. Steve Phinney as well as Jimmy Moore and Dr. Adam Nally are my heroes in this from now on.  I was on a keto group online and one woman asked if eating 800 calories a day was okay or not. People were congratulating her and told her to keep it up. Say what?

- Is there a happy medium for pregnant moms on eating carbs?

Hey Jimmy and The Doc,

I know you've talked before about pregnancy and that babies are born in a ketogenic state. I've got a question with a slightly different slant for you on this subject.

I had my first baby six years ago before I started eating low-carb. I had gestational diabetes during that pregnancy and I naturally started lowering my carbs to control my blood sugar. I was scolded at the time by my doctor and was told I can't eat low-carb during pregnancy. I was sent to a nutrition class and told to eat my "healthy whole grains" in moderation.

Fast forward, I have been low-carb for the past 3 years and have been more ketogenic for about 2 years. My blood sugars have been improving over the past year and I am now below that pre-diabetic level. Now I am now 10 weeks pregnant with baby number two.

I know of plenty of anecdotal stories about heathy babies born to ketogenic moms and I would like to stay low-carb because I feel like whatever keeps me the healthiest will keep the baby the healthiest. And I know I am extremely carb sensitive and I'm afraid that if I start adding in carbs to around 50g, it will raise my blood sugars and probably push me into a gestational diabetes again.

So my question really centers around the health of my baby. I understand that babies born to women with gestational diabetes are more at risk for developing diabetes. Chris Kresser has mentioned that babies whose moms eat a ketogenic diet turn on the epigenetics to begin storing excess calories.

Obviously I want my baby to be as healthy as possible and don't want to pass along all the struggles with weight and diabetes that I have had to deal with. So where does the happy medium lie?

1. If I don’t sustain ketosis, am I getting the full benefits of being keto-adapted?

Hey guys,

I’ve been on and off the keto train for a few years now. My problem is I have a hard time staying in ketosis for longer than a couple of weeks. I still have a few carbage loves that I can't seem to shake. I get back into ketosis about 36-48 hours after falling out, but then only stay in it for a short time. Do I still get the positive effects of being in ketosis even if it is for a short time? Or have I never really switched over to becoming a fat burner? I love Keto Talk and listen to it every week. Thanks for all the info you provide!

2. What’s the purpose of exogenous ketones products like KETO//OS if the ketones aren’t sustained?

Hi Jimmy and Adam,

I am a 250-pound female, overweight my whole life, 57 years old, and have finally had enough of it. I decided to go on a ketogenic diet one week ago and have already lost over 10 pounds. I feel good, have more energy, and experience mental clarity. I have read many books and feel like I know what I am doing, although I am still not in ketosis yet.

I have been taking the Pruvit product, KETO//OS, and the urine ketone sticks turn purple a couple of hours after drinking it—but it doesn’t last long. I am taking two servings a day, but I don't understand the purpose of this product. I thought perhaps it would help me get into ketosis and stay there along with following a ketogenic diet, but that's not happening. I am not eating more than 20g carbs a day.

Any advice or direction you can provide would be greatly appreciated. Thank you!

3. Is there something about keto that leads to severe upper abdominal pain?

Hi Jimmy and The Doc,

Several months back, I started the keto diet and my husband followed suit. He felt great for the first month and lost about 16 pounds. However, around week six, a big problem emerged for him. About 1-2 hours after eating, he would develop a severe upper abdominal pain that would last for at least two hours. The pain was in the center of his body around the diaphragm area. Sometimes the pain was so debilitating that he could barely move. 

We took him to the doctor for a full workup. They ran a gallbladder sonogram and function test, a liver enzyme test, and a GI scope—all came back normal. Ultimately he had to stop keto. As soon as he did,  the pain left.  When he has subsequently had low-carb meals, the pain returned with a vengeance. Any ideas why eating keto seems to bring this on and how he could prevent it? He would love to resume eating this way again if he could figure out this problem.  Thanks so much!

First off let me say you guys and your show rock. You guys are really helping people and changing lives so thank you for that. I've been following keto for 7 months now eating 20g or less carbs a day and haven't cheated once. I haven't felt the need to because I'm loving it, it's the best thing I’ve done for my health, and up until now I've lost about 75 pounds which is fantastic.

I'm going to Thailand in October for a couple of weeks and I know this is swearing on your podcast—but I want to cheat! It’s not because of social pressures or anything. It’s just something I want to do for two weeks on my holidays trying all the new things and having a few beers with my friends.

As soon as the holiday is over I'll be back on keto before the plane touches down because it's amazing. What should I do to prepare for eating some carbs for those 2 weeks? I've heard you guys talk about swelling and kidney stones and I'm worried I might have an upset stomach for a few days when I return. I want to enjoy my holiday and everything, so what’s the best way to go about cheating to minimize the side effects?

- Did my ketogenic diet lead to the development of kidney stones?

Hello Jimmy and Adam,

I have been eating keto off and on for the past three years. In April, I got really sick to my stomach for a couple of days and started to get a strange pain in my left side. It got progressively worse until I had to go to the doctor about it. By this time I could hardly talk because the pain was so intense. The doctor said I had the most kidney stones he’s ever seen for someone so young and I have a family history of kidney stones. Because many of them were so large, he said I’d need to undergo treatment to have them removed.

Ever since this incident with the kidney stones, many people have told me my ketogenic diet did this to me. I don’t believe that after reading Keto Clarity and other resources on ketosis, but I would love a more elaborate explanation to give to these "negative" people in my life. My doctor said my diet doesn’t matter and that I need to drink a LOT more water to prevent this from happening again. Did keto do this to me? And what can I do to help prevent this from happening again? Potassium? Fermented foods?

1. Can consuming more dietary fat offset a carbohydrate indiscretion to remain keto?

On one of the Facebook groups I'm on, people are often posting pie charts of their macronutrients to show what percentage of fat, protein, and carbohydrates they are consuming. One of the recent entries was from one who claimed they balanced out a slice of wedding cake they consumed by drinking a half cup of olive oil to keep themselves ketogenic. After reading Keto Clarity, the takeaway I took from that wasn’t necessarily about percentages as much as it was keeping carbohydrates low to your tolerance level, not exceeding your personal protein threshold, and then eat fat to satiety without necessarily worrying about calories. So how can this person justify drinking more fat to offset the wedding cake? This doesn’t make sense to me.

2. How do I keep from losing too much weight while eating a ketogenic diet?

Hi Jimmy and The Doc! I've recently discovered your podcast and have been binge listening to get caught up.  They're extremely informative and a great resource! Thanks for all the great info!

I was wondering if you guys have any advice on maintaining weight while following a keto lifestyle. I’m 42 years old, female, 5'6" and 125 pounds. I've struggled my entire life with a low appetite and low weight and can only easily put on weight when I eat "carbage.”  My physician has recently prescribed Topamax 50 mg for migraines which makes the poor appetite even worse. I've been following a traditional Atkins diet for many years and recently tried a switch to keto to help transition my body composition from mostly fat to lean muscle.  The problem is with my already poor appetite, I just can’t seem to maintain my weight and the pounds are dropping off me at an alarming rate (down from 125 to 118 in one week).  

I have a crazy, busy lifestyle which I'm sure doesn't help. As a Physical Therapist and my typical day starts between 3:00-3:30AM with 1-2 hours of of cardio and weight lifting followed by a 10-12 hour workday 5 days/week.  I tried putting coconut oil in my coffee and making fat bombs, but the lack of appetite and busy schedule are still working against me.  I'd like to try keto again but I don’t want to lose any weight in the process.  Suggestions?

- Are there any medications keeping me from losing weight on keto?

Hey Jimmy and The Doc,

Thanks for the awesome podcast, I really dig it. I started eating keto a couple of months ago and lost about 10 pounds right away. But I’ve been stalled ever since. I’m wondering if you know of any medications that might be preventing me from reaping the full weight loss benefits I should be seeing?

Specifically, I’ve been on a statin since 2008 when I had an A-Fib episode and an angiogram showed plaque. My doctor is very reluctant to take me off of it due to my family history of cardiovascular issues. I'm also taking medications for heartbeat regulation (cardizem), high blood pressure, and a baby aspirin. I was diagnosed as pre-diabetic last November and can't wait to retake my blood tests in a few weeks. I feel great and my clothes are fitting me better, but I'm still only down 10 pounds.

Any advice would be great. Love the show and thanks again!

- Will keto help with my Heriditary Angioedema (HAE)?

Hey Jimmy and The Doc,

I have a tough question for you guys to answer. A few years ago, I was diagnosed with an incurable, scary condition called Heriditary Angioedema (HAE) and have a mutated C1-inhibitor gene. Apparently I lack some good enzymes and this has caused quite a lot of digestive issues. When an attack is triggered, blood rushes into my intestines, causing swelling and excruciating pain. My body then turns on itself with hives, vomiting, diarrhea, swelling of lips, ears, and tongue, with the possibility of my throat swelling shut.

When I turned to Paleo and started a fitness regimen a couple of years ago, I lost 60 pounds, healed my adrenal glands, and seemingly put my HAE into remission. Now I am using nutritional ketosis and doing very well with it. But last night I had an attack which is the first one I’ve experienced in a couple years. I know with ketones they can strip your body of good bacteria and possibly make me more prone to bad bacteria. My condition can also be triggered by the presence of H. Pylori. Can you share any experience you have with this? Should I be concerned about being in ketosis with my specific condition?

- Detecting diabetes’ deadly ketones

1. Why are my blood ketones lower than I expect them to be?

Hello Jimmy and Dr. Nally,

I started a ketogenic diet in January starting at 388 pounds. Everything in my blood work and health markers were normal when I began and I’ve lost well over 60 pounds and counting. I was using Ketostix to test for ketosis and suddenly noticed they weren’t changing to purple anymore after a couple of months. Because I wanted to make sure I was in nutritional ketosis, I bought the Precision Xtra blood ketone meter. My first blood ketone test registered in at 0.6! I was pretty upset at that because I had kept my total carbs under 20 grams daily the entire time I’ve been eating keto. I’ve since seen my ketones rise to 1.2 but then they drop back down to 0.6 again. As you can imagine, I’m kinda stumped at how this can possibly be. Is this normal? What can I do to improve this?

2. Is it a good idea to continue taking a statin medication while eating a ketogenic diet?

Hey Jimmy and Doc Muscles! I’m a faithful listener and fan of the work you are doing. Here’s my story: My father-in-law is on a statin medication for his high cholesterol and seems interested in going on the ketogenic diet. I obviously don’t think he needs to be taking the statin when he’s on keto, but I'm not sure if that’s a good idea or not. Is it safe to be in ketosis while still taking a statin drug for a bit longer before discussing with your doctor about getting off of it eventually?

3. If I’m very active, what macronutrient do I eat more of to remain in ketosis?

Hi guys,

I love your Keto Talk podcast. Here's my question: my goal with keto is to shred fat but I don't want to necessarily lose weight. In fact, I'd actually like to gain 10-15 pounds eating this way. I have a very active job walking an average of 25,000 steps per day and I also strength train 4-5 days a week. My average daily calorie burn is around 3,500 calories as a 170-pound man. Because I workout hard and have a very active job, can I eat more carbs? If not, then what do I eat more of?

I’m a Type 1 diabetic who eats keto to control my blood sugar levels. My husband has been envious of the foods I've been eating and wondered if it would be beneficial to him. He is 6'4" and very slender at 180 pounds with an incredibly fast metabolism and a bottomless stomach. He has always had incredible blood sugar readings getting 80-90 readings consistently after eating high-carb meals. So my question is this: Is a ketogenic diet a good option for people like my husband who are healthy, with no insulin resistance, or carb sensitivity? I can't tell you how much I love this podcast and look forward to it every week! My question is whether you can undereat on a ketogenic diet? I've been keto almost two years now and I started working out more, but I'm just not hungry enough throughout the day to eat any more than 1000 calories a day. I know the keto way of eating is satiating, but do I need to push myself to get in more calories? I have gained weight, been tired, and it takes me forever to heal muscle pain after a workout. Any advice would be greatly appreciated.

2. Do varying levels of blood sugar negate the benefits that come from nutritional ketosis?

Hi Jimmy and Doc Nally,

I have listened to all of your Keto Talk podcasts and I am always anxious to listen to the next. Awesome job you guys. Keep up the great work. I am a "health nut", not always living healthy but always wanting to learn and live the best healthy lifestyle I can. I am 52 years old, about 190 pounds and 5'10" tall. I started eating keto with intermittent fasting about 6 weeks ago and am loving it.

This week, I purchased glucose strips to start measuring my blood sugar as well, which has lead to some confusion. My glucose averages less than 81 (4.5 mmol), but on a few occasions as low as 61 (3.4 mmol) and I feel great. On the few very recent occasions that I have tested my glucose and ketones at the same time, I don't really see a "strong" correlation between the two. For instance my glucose could be at (4 mmol) and my ketones at (2 mmol); then another time my glucose still at (4 mmol) and my now ketones at (4.5 mmol).

My question is this: even though my blood ketones are at 4 mmol, am I still getting the therapeutic effects of being in ketosis regardless of where my blood sugar level is at? Is it a matter of just having the ketones present or are the benefits relative to the glucose concentration when it comes to efficacy?

In one of your recent podcasts, The Doc mentioned that you don't need to have surgery to have excess skin removed after losing a large amount of weight. Is that true for all forms of weight loss or just something with the ketogenic diet? I've lost over 100 pounds in six months going from 300 pounds to my current weight of 194—most of it happened with calorie restriction.

I still have quite a bit of excess skin and I'm wondering if a ketogenic lifestyle could have helped me avoid that. All that said, I have been on a ketogenic diet for the past several weeks and have no plans of ever stopping thanks to the raised energy levels, mental clarity and improvements to my arthritis. My biggest wish is that I discovered ketogenic diets back in February when I started this!

- Will the body ever be able to handle more carbs and protein again after eating ketogenic for a period of time?

Love the show. I'm catching up on all the past shows and just finished Keto Clarity. I've transitioned from 20 years eating a low-fat, high-carb vegetarian diet. My body fat crept up to 35% and I've tried cutting calories, cutting fat, cutting dairy and never achieved the lean body I'm after…plus I was hungry all the time. I switched to Paleo for the last year at about 60g "net" carbs per day but didn't lose weight.  Now I realize I was consuming too much protein for me and 60g net carbs is also too much. I suffered from TERRIBLE sugar cravings and I'd succumb and end up binging on candy for a day then feel bad about it.

Now I've been following a keto diet of 80% fat, 14% protein and 6% carbs for the past 3 weeks. I have zero sugar cravings, but I'm not yet fat adapted. Fortunately, you've armed me with the knowledge that this will take several weeks so I'm being patient and sticking with it! I understand that the body can be mysterious and tricky. So my question is this—will my metabolism alter itself throughout the years in a way that will make it harder and harder for me to stay in ketosis long-term? In other words, does the body ever evolve to have lower thresholds for carb/protein intake after years of being ketogenic that would subvert my best efforts to maintain ketosis?

I've been doing keto for 2 months now and was wondering if there is a suggested percentage of saturated, monounsaturated and polyunsaturated fat that you would suggest that should make up my % for the day? It seems like I'm very high in saturated fat and low on the others. I’ve heard from others online that it should be 20% saturated and 80% poly/monounsaturated fats. What say you?

2. Is there a keto-friendly IV solution when you are in the hospital?

Hi Jimmy and Doc Muscles,

I'm a subscriber, and listener since Episode 1. About a month ago, I had an emergency hospitalization with my gall bladder. I’ve been ketogenic for 18 months and getting amazing results. While I was in the hospital, they hooked me up to a glucose IV for the five days I was there. My records show my blood sugars got as high as 175. But being insulin resistant with glucose running 24/7, I suppose that's not a surprise.  I could tell I was on glucose because I got hungry and pretty much stayed hungry until I was home for a few days and eating low-carb, high-fat again.  Is there an alternative us Ketonians can ask the hospital for?  I don't suppose they make IVs of exogenous ketones or beta-hydroxybutyrate do they?

3. Can you assume you have high insulin levels based on high blood glucose and low ketone levels?

Hi Jimmy & Doc,

Have either of you measured your insulin levels directly both in ketosis and out of ketosis, to verify:
1) that your insulin levels are too high out of ketosis (i.e., hyperinsulinemia / insulin-resistant), and
2) that being in ketosis normalizes the insulin levels?

Because there are many hormones and other factors at play, it strikes me that indirect markers of insulin levels such as blood glucose and BHB levels are potentially a weak proxy, at best. It would be like assuming that everyone who is excessively tired must have adrenal fatigue, when in fact there are many other possible causes for that symptom. If you haven't done the direct testing of insulin levels for your individual cases, then are you not merely operating on educated speculation?

I fully understand that testing insulin per se is more difficult and much more expensive than testing BG or BHB. However, I am concerned that the widespread assumption of elevated insulin levels in discussions around weight, diet, carb levels, and more has become an across-the-board reflexive answer, rather than an empirically confirmed cause in individualized cases.

Thank you so much for your podcast.  I have listened to about half of them and enjoy both the information and your synergy and humor. I have been on a ketogenic diet for 4 months and am happy to report I’m improving my hip to waist ratio and decreasing my disease risk factors.

I am curious how someone could determine how insulin resistant they might be?  I understand from listening that you both know you are very sensitive.  What are the indicators that help people figure this out?  And do epigenetics play a role such as the MTHFR gene mutation?

My girlfriend went to her best friend’s house and her family had made a huge homemade pizza. She felt like she had to eat what was put in front of her and she made the choice to just eat the toppings. But she felt like she was going to be judged by not eating everything and thus offending these people she loves. What’s the best way to deal with this kind of situation? - Does the kind of carbohydrate you consume matter in limiting your intake while pursuing ketosis?

Hi Jimmy and the Doc! A carb is a carb is a carb...or is it?! I have been following keto for about a year now and have actually seen a large number of my friends join in and start eating ketogenic as well. However, we have found that there is a slight difference between us. Some of us have noted and realized that we are so insulin resistant that we keep need to keep our carbs around 20g daily while others of us can consume upwards of 40g and still be in ketosis. One person in our group has stated that eating carbs, whether it is from sugar or bread or vegetables, all turn turn to sugar in your body. So she claims she can fulfill her 35g of carbs by eating M&Ms if she wants to and it will still process in her body the same as if she were eating 35g of vegetables. I'm not sure that I believe this but I am curious what the implications are on the body when eating simple carbs like sugar, fake sugars, and flour vs. getting them from complex sources such as leafy greens and other types of low-carb fruits and veggies.

- Is it detrimental to fat-adaptation to come out of ketosis temporarily from time to time?

Hello Jimmy and the Doc! Can you be fat adapted and get kicked out of ketosis? I have been keto and fat adapted for over two years. I have lost 108 pounds and I lift heavy. Sometimes when I overdo the protein or eat enough berries to increase my carbohydrate intake, I measure my blood ketones four hours later and they’re 0.2 -0.4. Then it takes me 12- 24 hours to get blood ketones back above 1.0 mmol again.  So, am I still keto-adapted? Do I need to be worried about getting kicked out of ketosis like this?

- How does someone with hyperinsulinemia deal with lower ketone levels?

Hi Jimmy and Dr. Adam,

My wife suffers from hyperinsulinemia and struggles to maintain a blood ketone reading of 0.6 mmol using a combination of intermittent fasting and eating within a 4-5 hour window. Her blood sugar reading average 4.4 mmol/L (80 mg/dl). From my personal experience, I know she’s not feeling the full effects of higher ketone readings such as mental clarity. Do you guys have any strategies for boosting these lower ketone readings in the presence of hyperinsulinemia?

I have been following a ketogenic lifestyle for almost a year—and there’s NO going back! Since last summer I have improved my health markers and lost 35 pounds (with 50 more to go) and have NO cravings. But my weight loss has stalled out since Christmas and your various podcasts & periscopes keep me inspired to NEVER eat the carbage on a regular basis again. One thing that still plagues me is my diet soda habit. Nothing else “hits the spot” like my Diet Dr. Pepper and I seriously need help in forming a strategy to get rid of the stuff. What exactly is so harmful about the diet sodas and how might that affect my weight loss on keto? What can I drink instead that might hit the spot since I’m not really a coffee or tea drinker.

2. Does a ketogenic diet lead to insomnia and how can I overcome this?

Hi Jimmy and Adam,

Thank you for all your work in endorsing the keto way of living! I am having issues with maintaining ketosis due to insomnia, a racing mind, and loads of energy at bedtime. I've been Paleo for 3 years now but am still struggling to lose belly fat. I need to drop at least 8kg (18 pounds) to be at my ideal weight and I haven't eaten bread, pasta, rice, sugar, fruit or sweet drinks for over three years. But I’m still struggling to lose that weight.

My question for you guys is how do I overcome the insomnia that seems to be caused by being in ketosis? I love my mental clarity and energy when in keto but it doesn't last long because of the insomnia. I have tried 1/2 teaspoon of honey, 600 mg magnesium glycinate, and 1500mg L-Tryptophan at bedtime but all to no avail.  So I end up having a piece of my Paleo bread (10g of carbs) at 2am so I can get to sleep. I’ve always been a good sleeper before attempting a ketogenic diet.

Any suggestions in overcoming this would be great.

3. Would former contestants on The Biggest Loser be able to heal their bodies on keto?

Hey Jimmy and The Doc,

I’ve been reading a lot about the former contestants on The Biggest Loser gaining back their weight and messing up their metabolism even more than before they went on the show. Would these people benefit from eating a ketogenic diet? If they are constantly struggling with hunger, I would think eating low-carb, high-fat would be invaluable to them because it will satiate that hunger.

I read the description of what one of them was eating for breakfast: "one egg and two egg whites, half a grapefruit and a piece of wheat toast." No wonder he's so hungry! He's basically eating sugar for breakfast. I would be hungry about 20 minutes after eating that and never feeling satiated. Why has no one recommended a ketogenic diet to them?

As for their slowed metabolic rate, I am not sure what to think. Is that something they can overcome or reverse? Or is it permanent? Does that happen to everyone who tries to lose weight or only those who take the very extreme and unsustainable approach The Biggest Loser takes which I would think would be massively stressful to your body and soul? How does that work and can what you eat to improve your metabolism?

Hello Jimmy and Doc Nally. First I wanna say I listened to about 10 episodes today. I am so into what you guys are doing here. I was diagnosed as hypothyroid four years ago and recently diagnosed as diabetic with a fasting blood glucose of 254. I started keto in April and I’ve already gotten my A1c down to 6.1.

I've been checking my blood pressure and my readings have been over 150/90 several times. I can feel my arms and feet get numb throughout the day and I'm freaking out. When I eat half a grapefruit and tested again, I had two readings under 120/60.

Am I doing something wrong with keto? I listened to several podcasts episodes and couldn't seem to find any information about blood pressure side effects. When I test with Ketostix, it shows I am in ketosis. My blood sugar has not risen above 100 mg/dL, so why is my blood pressure so high?

Hey Jimmy and Doc,

Just listened to your new Keto Talk podcast and what a great show you guys do. You mentioned pancreatitis in a recent episode and I wanted to share an experience I had with this. I cheated on the diet with lots of carbage and sugar. The next day I experienced an acute inflammation of the pancreas. It wasn't a full blown case of pancreatitis but it was painful and lasted a few days. It also felt like I was having some gallbladder pain. My lipase level was 40 which was still within normal but still high. I can see how someone who was prone to this could easily have a severe reaction resulting in hospitalization. I have decided being on the ketogenic diet isn’t something to play around with. Now I'm all Keto all the time. I guess insulin resistance stays with me even though I lost a lot of weight. Thanks for taking on the tough areas and having great topics that are relevant in the real world. Thanks for your podcast!

I think I heard Doc Nally say that going on a ketogenic diet increases testosterone in response to someone asking if ketosis can promote muscle building.  But isn't increased testosterone less than ideal for women because it promotes the development of PCOS? Or is the increase in testosterone levels while in ketosis only something that happens in men?

2. Why am I still struggling with low energy and low ketones after months of being in ketosis?

Hey Jimmy and the Doc, I am a 40-year old woman and I have been in ketosis for 5 months now. I believe in this way of eating, but am still struggling with spells of low energy and low ketones. I really want to experience the endless energy and mental clarity that everyone talks about. My ketones remain around 0.5 mmol and never higher. I still don't feel great on keto like I had hoped. On a positive note, my cravings have now gone, but I really want to feel better. 3. Can being in nutritional ketosis above 1.0 mmol cause painful headaches?

After 12 weeks on keto, I have only lost 15 pounds and 2.5% body fat. While I’m not pleased with this slow progress, I’m even more concerned with the massive headaches I get when I go over 1.0 mmol on the blood ketone meter. What is it about ketosis that could be causing these headaches?

I have been on and off a ketogenic diet for about two years now and I have been back in ketosis for four weeks now. I’m determined to stick to it and have finally taken the plunge by giving up the artificial sweeteners. This means no sugar-free gum, no sugar-free sweets and the hardest thing of all for me is no Diet Coke! However, I am still using Stevia. I heard that just eating something sweet causes insulin to be released. Is this true?

5. Is my ketogenic diet causing me to cramp up before and during my half marathon racing?

Hi guys,

I'm a 43 year old triathlete who's been on low-carb and keto for a couple of years to increase my endurance. I'm quite happy with my performance during training, but I am consistently cramping during my most recent races up to a half ironman distance. There are two different things I do prior to racing: 1) 12-24 hours before my race, I increase my carb intake from less than 50 g a day with 2 additional small sweet potatoes. Then I switch back to next to no carbs for the remaining 12 hours prior to racing. 2) I take 2 puffs of Ventolin inhaler as a preventer 30 mins before racing. I've had a couple of race-induced asthma attacks and they were not a pleasant experience so I tried adding the inhaler as suggested by my GP.

The issue is I'm cramping up badly even during warmups. I tried increasing my salt intake and magnesium glycinate supplement leading up in the weeks before racing. I'm concerned that it's a combination of keto and the inhaler causing these issues. Do you have any ideas for me to prevent these cramps? Keep up the good work.

6. Is MCT oil a better fat to use on a ketogenic diet than other fats like coconut oil, cream, or butter?

I hear a lot of debate amongst people I know who are keto about which fat is best to use, especially in coffee or hot tea. Everyone has their own opinions about coconut oil, heavy cream, butter, ghee, MCT oil, avocado oil, etc. I am hearing from a lot of people to skip the coconut oil and use MCT oil instead. While I do have MCT and use it occasionally in tuna or on salads, now I'm wondering if I should use it everywhere and skip the other oils like coconut and avocado oil. What is you take on this for us Ketonians? Do you guys think there is an added benefit in using MCT over other fats or is is merely personal preference?

7. Why do I have a constant stomachache while I’m on a ketogenic diet?

I'm about 3 weeks into this ketogenic lifestyle and I've been experiencing a lot of stomachaches. I'm assuming this is because I came from a low-fat way of eating prior to this. Is there a way to help with the transition or is this pain just something I need to learn to live with?

8. Do you have to be in ketosis to burn fat?

Thanks so much for the helpful information about keto that you discuss on your podcast. It has been a great help to me since my doctor put me on a low carb diet due to early signs of insulin resistance. I have a question that I’m still unclear about. Do you have to be in ketosis to burn fat?  I've heard a lot of discussions that you are automatically a fat burner when you consume 150g or less of carbs daily. There seems to be mixed messaging from lots of different people on whether or not you can burn fat outside of ketosis.

9. Does being in ketosis lead to daily spotting and extended periods?

Hi Jimmy! Long time listener and loving the new show with Dr. Nally! I got inspired by your N=1 experiment with nutritional ketosis and went full keto improving my sleep, eliminating my cravings, and full of energy. It was a miracle diet for me and everything was perfect except for when I started to have spotting/on my period every single day. I let it go for four months because I just felt so good in ketosis. I contacted you and you connected me to Jackie Eberstein who recommended I increase my carbohydrates in 5g increments until the issues resolves itself. I didn’t want to do that at the time since I knew the cravings would come back. After seeing my gynecologist who wanted to put me on hormone medications, I instead binged on carbs and fell down that rabbit hole losing my period for the next year. Am I the only one this has happened to? Why did this happen?

You and Doc Muscles keep up the good work. You are changing lives and Jimmy you were the first low-carb person I ever listened to! Thank you for all you have done.

10. Are ketones in my baby’s breastmilk safe for her to consume? And why did my milk supply drop when I went keto?

Hi Jimmy, I’m an avid listener of all your podcasts, but I especially like Keto Talk. I gave the ketogenic diet a shot last year before I got pregnant with my second daughter and I loved how I felt on it. I had a difficult pregnancy and wasn’t able to continue the diet, but now that my daughter is 3 months old I’m ready to do something to lose the 20 pounds of baby weight that I have left to lose. I’m breastfeeding still and I just want to make sure that ketones in my body and in my breastmilk will be safe for my baby. Dr. Atkins recommended in his book for breastfeeding moms to consume a maintenance level of carbs while pregnant and breastfeeding.

Additionally, my milk supply when I started low-carb (but not ketogenic) after the birth of my first child significantly dropped. At the time I didn’t know that I needed to add salt and other minerals to my diet when reducing the carbs so that may have solved the problem. Before I go full-on ketogenic this time, I am wondering if there is anything else I can do differently to maintain my milk supply this time around. Looking around the Internet, I found this to be a common concern. Has The Doc encountered this with any of his patients?

11. What is the impact of the supplement creatine on ketones, blood sugar, and insulin levels?

Hey guys! I am a big fan of the two of you and information you provide! I have a question today about creatine and ketosis. I lift weights 5 days a week and love it. I focus more on powerlifting than traditional bodybuilding and have seen great results the last four months switching over to a ketogenic diet. Before I dialed in my protein consumption to get into ketosis I always took two supplements: creatine and whey protein. I still take some protein right after a workout (maybe 15-20 grams) but nothing like I did before learning about protein's impact on ketosis.

With my protein consumption dialed in, I wanted to ask how the consumption of creatine impacts ketosis, glucose, or insulin levels? I typically have 7-10 grams of creatine per day. My ketone levels and glucose appear to be fine, but I still wanted to make sure there are no adverse effects I should be aware of as it relates to ketosis.

Hi guys! I'm new to keto and am LOVING this podcast! Thank you so much for taking the time and effort to do it! I want to start my ketogenic diet by easing into it as a way to avoid experience the pain associated with the ”keto flu." I’m looking at it like a drug addict coming off the drug they are addicted to, I’d like to wean myself off of sugar and carbs. Is there any reason why doing this would be a bad idea?

I’ve followed a ketogenic lifestyle for 4 years now. My WellnessFX Cardio IQ blood test shows I have excessively high cholesterol (LDL-P 2478, Small LDL-P 498, triglycerides 98, HDL 76, hsCRP .02, Lp(a) 207, homocysteine 13.8, fibrinogen 175). Using my 23andme raw data, I discovered I'm not a good metabolizer of saturated fat. In fact, it claims that a ketogenic diet would be dangerous to my health. Will eating more fish and seafood, avocado, nuts and seeds, olives and olive oil work to deal with this while I try limiting red meats, eggs, butter, and coconut oil. I am normal weight and otherwise in good health.

I heard a guy named Ian Ryan from the Fearless and Healthy podcast say that eating too much healthy fat will still make you fat.  He said this on another podcast called The James Swanwick Show.  Is it possible to go too crazy on fat and still get fat? I think I already know the answer, but I am still curious.

2. Can you eat a zero-carb, high-fat diet and still be ketogenic?

I recently stumbled across an article about some people who switched over to a zero carb, high-fat diet which eliminated plant foods entirely. Their diet consists of almost entirely beef products rounded out by salmon, eggs, bacon, cheese and butter. My question is this: If you followed a ketogenic diet from a macronutrient standpoint, is this a sustainable way of eating. How would you be able to derive enough vitamins and minerals from the meat products that you could omit all plant foods?

I am a wellness chiropractor in Southern California who has been challenged by weight and blood sugar. From listening to your Keto Talk podcast, it sounds like you and Dr. Nally believe that the ketogenic diet is a permanent way to eat.  He says that he has been eating this way for over ten years.  Having only done this way of eating for a couple months, I have no idea about the long range necessity of it for me. But it would seem that our ancestors who lived before refined sugar and high fructose corn syrup did pretty well on a diet that was much higher in carbs. To me it seems who many people like me who have a damaged metabolism are eating keto as a means to heal. But it would seem like eventually a person could go back to eating more carbs and not gain weight and not throw their blood sugar out of balance. What is your experience with people who go on the keto diet, lose weight, get off metformin, balance their blood sugar and then after awhile try eating more natural carbs like fruit and some whole grains?  Does this happen or do people just need to stay on this diet indefinitely?

I’ve been following a keto diet for a few months now and feel great. I’ve read Keto Clarity and listen to all of your Keto Talk podcasts. My question about my brother who is overweight and eats the SAD diet. A recent trip to the ER led to the discovery that he now has gallstones and the doctor is recommending removing his gallbladder all together. This raised my curiosity about how a ketogenic diet would affect his gallbladder. Is it possible that starting keto for him right now while he still has his gallbladder could help the gallstones or make them worse? I’ve also researched that fasting can increase gallstone production due to the decreased need for bile during the fasting period. Because many of us Ketonians intermittent fast as part of our lifestyle, I’m concerned this could be a problem for my brother as well.

Do you know if there is a relationship between very low-carb diets and Candida gut bacteria? I am suspecting I may have Candida and I have read online from people like Chris Kresser and Paul Jaminet that my very low-carb diet could be an issue. They make the claim that Candida feeds off the ketones. My diet is not necessarily ketogenic, but I’m sure I generate more ketones than the average person. So I’m not sure if the diet aspect would even affect me. My blood ketone levels are around 0.3 on average, which is below the level of nutritional ketosis (I can’t cut my protein as much as a ketogenic diet requires). This information is out there on the Internet and I’d love to have your take on this topic.

- Is there any such thing as too high ketone levels?

I have been strictly keto for almost 3 weeks (between 75-85% fat, around 5% carbs and between 10-15% proteins). I’m a bit concerned because my ketones levels have been between 4.5-6.3. And my glucose levels are 53 and 80.  My purpose in being ketogenic is to lose weight. As I understand it, if we go over 3, then you can’t lose weight. But, is this dangerous? How can I be between 1.5 and 3? Should I eat less fat? Should I eat more carbs? My eating schedule is eating in the morning (usually 1 can of sardines in olive oil and tea with 1 tbsp of butter and 1 tbsp of coconut oil). Then, around lunch or a bit later I eat fat again (usually 2 tbsp) and then eat at night. I do not count calories and eat until I’m satiated. Thank you for reading my mail and your response. It is great to have someone that I trust to ask my questions. It gives me courage.

My mother is 75 and in pretty good health overall. High blood pressure medication and about 25 pounds heavier than she would like. Is keto safe for this generation?

3. Can a keto approach work even if you can't exercise?

I have recently developed a heel spur on my right foot and arthritis in my hips. Only being 50, this is very discouraging as I have been exercising regularly the past 4 years to help keep my weight and size down. I am not able to do this now. Will this eating plan help with arthritis and enable me to still lose weight even though I am more sedentary?

I would like to get Doc Nally’s input on Dr. Chris Masterjohn’s talk at PaleoFX this year. Chris discussed the impact of long-term low-carb diets on the sex hormones and thyroid hormone via the role of insulin on these hormonal pathways. He outlined that insulin plays a significant role in the production of thyroid and sex hormones in signaling to the body that we are in a sufficient state of energy to be reproducing. This is why he believes there is great risk of low sex hormones on long-term low-carb nutrition. Any thoughts?

- Why did I lose weight fast in my face, but not in my stomach on a ketogenic diet?

I have been in ketosis for about 5 weeks. Basically my face fat is a lot less than my body fat.  So i still have a nice sized gut but my face is pretty skinny. A lot of paleo are experiencing this as well. I’ve heard some people have meaty faces and some have more boney (maybe genetics). Koreans seem to have a nice big fat face and like 0% body fat, how are they doing this?! People in my life, friends and family are saying I'm too skinny, but it's my face not the body.  wondering if Doc Muscles has any ideas…maybe mineral deficiency or protein too low? I'm 80% fat, 15% protein and 5% carb, calories are anywhere from 1300-2000. How can I make my face fatter?

I got in ketosis and stayed there for a solid month testing with the Ketonix breath analyzer, blowing consistently yellow and red, with an occasional green. I felt drained, exhausted, and weak for a whole month.  I used the KETO//OS product and ate good fats and moderate protein.  I used sea salt often hoping that would help. What was I doing wrong?  In order to get through my busy days and start feeling good again, I upped the carbs knocking me out of ketosis.  I'm still low carb, but not ketogenic.  I have energy again.

1. Does being in ketosis repel mosquitos?

Hi Jimmy and Doc Muscles,

So, I sort of recall hearing somewhere someone said that mosquitos like "sweet" people.  That is, people with lots of sugar in their blood.

Would that mean that they wouldn't like people in ketosis? 

Is there any research out there on the topic?

Hello...help please! I eat around 40 carbs total and from veggies each day, and have had improved health and stable weight and mood for two years doing so; my Paleo trainer (intelligent, thoughtful guy) advises me to eat tubers some each week so that I will be "metabolically flexible"- so my body is accustomed to burn both glucose and fat easily; he says I'm losing my ability to burn sugar easily by being consistently low carb.  He says just as my body had to learn to be fat adapted....now it is not "sugar adapted", and this is making me even more intolerable to starchy/sugary foods...equating to less overall health. Have you heard of this "flexibility factor"?  Can you and Doc Nally address this in a podcast soon.

Does Dr. Nally have any experience with normal weight patients who are perimenopausal or menopausal age who adopted a ketogenic diet to alleviate the hormone rollercoaster and to head off the potential weight gain that often happens during that time of life?  I wonder how a diet that is great for fertility in women of childbearing age could also be good for older women in which increased fertility and higher estrogen levels increase their cancer risk.  Any thoughts would be most appreciated.  Keep up the great work!!!

- How can I get back started on keto again?

I need suggestions and tips on getting back into ketosis after a period of time of not doing it. I was actually keto (no dairy) for over two years and then a practitioner strongly recommended I give raw dairy a try. I did it for a few months and then realized I'd completely lost it. All the cravings, pain, sleep issues and weight (back up to 180lbs) came back and I couldn't get off the dairy (then other junk started creeping in and I might as well be back on the Standard American Diet)!

I had no cares in the world on keto. No cravings, no issues, just just feeling amazing all the time. Initially, I was about 210 pounds and went down to 145 in seven months, through the dietary changes alone. I regret not listening to my body and accepting that no dairy means no dairy and I realize it was my choice to run the experiment, but it was a huge fail and I have been on the off again on again keto rollercoaster for over a year.

So, to finish the question, are there any "tricks" that I can employ to get back to it, at least for long enough to reestablish the habit? Just jumping back in seems to be the best and worst advice I'm giving myself....

1. Do I need to quit keto once I hit my weight loss goal?

Question for you regarding Weight loss and Keto.  I am just starting on this keto way of living and eating.  I have lost 50 pounds in about 4.5 months.  Since I was at my heaviest weight though, I have lost around 80 pounds and I don't remember when I felt this good!  I need to lose more weight, a lot more weight, and I am wondering will my body just know when my weight is "right" and stop losing. According to my height, "the maximum weight I should be for a normal BMI" is 150 pounds, but I think that is going to be awfully thin for my frame.  Do I need to worry about losing too much weight, or will my body take control and stop losing weight when I reach my "ideal" weight?

2. Is flax and flaxseed oil okay on a ketogenic diet?

I LOVE the Keto Talk show. I listened to that yesterday on my lunch hour. May I submit a question? I am curious about your thoughts on flaxseeds and flaxseed oil. Are they okay for a ketogenic diet? I know the primary fat is polyunsaturated, and we are supposed to stay away from poly...but, I am not sure if that only applies to vegetable oils (canola, cheap cooking oils, etc.), and flaxseeds are supposed to have a variety of health benefits. Thanks!

3. What to do when you are sleepy on a low-carb ketogenic diet?

Hi Jimmy and Dr. Nally,

I have been on a keto/very low-carb diet for about 4 weeks now and every day when I eat my lunch, I am still getting sleepy.  It's fairly annoying!  It could be my job, but I am just wondering if there could be a reason.  My lunches usually consist of 5-7 oz of some kind of meat, 1TBSP of butter, along with several cups of vegetables that don't add up to more than 8 carbs.

I just did realize today that the coffee substitute that I've been drinking actually has about 2 g of carbs per teaspoon and I haven't really been counting the teaspoons, so I guess I am wondering if I am maybe in that "zone of misery" where my carbs might actually be higher than I thought or is it just that some people take a good long time to become fat adapted?  I do know that Dr. Nally said it could take 4-6 weeks to become fat adapted, so maybe that is it?

I just got a list of inflammatory foods to avoid from my acupuncturist and I was wondering why a lot of keto foods were on there. Specifically beef, dairy, avocado and nuts. I've been on keto for 3 months, I eat all of those foods almost every day. Also found it strange that rice was on the "okay" list.

Greetings!  I'm a 63 year young woman two months into LCHF and am slowing down on the weight loss at near 20lbs.  I've naturally fallen into a 16hr daily fast (sometimes a few hours longer) and am amazed at disappearance of an appetite!  I've been faithful to the diet as laid out by Dr. Eenfeldt's Diet Doctor.

I'm enjoying the podcast series with Doc Muscles.

There are many of us who entered adulthood when the grains were modified, healthy oils were switched out and the diet-heart nonsense and food pyramid were foisted upon us ~ and we bought it all.  Needless to say, we have much to undo, however age throws a curve ball but hopefully, it's never too late to live healthy.

Perhaps the Doc could address challenges faced by older men and women getting into Keto lifestyles (such as a slower result curve & why) and perhaps some strategies to keep the results on track.

I posit this is not a minority of your audience. We are the Baby Boomers after all and many are alert to the fact that our generation's overall health is in jeopardy.

My sister has PCOS (the advice she gets from her doctor is a big joke and no one has ever heard about keto and if they have they say that it's bad of course...!). I'd love to ask Dr Nally what he thinks is the best way to treat this condition. My sister is super thin although she eats A LOT! Is birth control pills really the only way to keep the cysts away or can a keto diet make her condition better?

- High dietary carbohydrate load could have detrimental effect on heart function

1. Does a ketogenic diet work well for explosive athletes as it does for endurance athletes?

Hi Jimmy & Doc Muscles!
I am a 28 year old female, in good health, and have gone through almost 2 years of my own health and fitness journey. I started a sport called Crossfit (I'm sure you've heard of it) almost 2 years ago.  Because my coaches are SO great, I haven't encountered any negative experiences with the sport that you often hear of. Instead, I have seen massive health benefits for my mind and body. Besides going from 28%to 22% body fat (I'm 5'3'' and 141 lbs), I've experienced greater stamina, mental clarity, sleep, and better performance at work as a traveling musician on the road. I've even competed as an amateur in 3 competitions.

I experimented with paleo-style eating, and even high carb low-fat (aaahh!), high protein, etc while being a Crossfitter, but recently I was on a tour overseas,  and living on a tour bus makes it hard to do my typical explosive, heavy-weight, anaerobic exercise. So I tried keto. About 3 weeks in I am seeing a 2.5 reading on my ketone blood meter daily, and I am feeling really good.

Here's comes the question, I promise.

I haven't yet had enough time back home doing Crossfit and Olympic lifting to see if ketosis is improving, worsening, or making my athletic performance the same. I know ketosis works great for endurance athletes. It even works well for bodybuilders doing hypertrophy-type training. But I am not doing either of those types of training. Instead, I do Olympic style lifts (requires explosive movement) and other VERY intense explosive movements like jumping up high surfaces, gymnastic movements, & all out sprinting.

Does a ketogenic diet work well for explosive athletes the same as it works for endurance athletes and bodybuilders?

2. Can I take a pain reliever like Aleve or Advil while eating keto?

I'm in ketosis. I normally don't take anti-inflammatory meds, but I tweaked my back and was wondering if IB or other NSAID's would bump me out of ketosis?

I am doing good on my low carb way of living and am now keto adapted. My question is this. I have been diagnosed with tennis elbow and have been prescribed Naproxen for the pain and inflammation. Is it ok to take or will it kick me out of being keto adapted. I would rather wait for it to heal slowly if that is what would happen. I don’t want anything to ruin the way I feel and the lost in weight that I am experiencing while eating this way. I eat 5% carbs – 15% protein – 80% fat. I feel so alive and energized eating like this. Could you tell me whether it is good to take the Naproxen or whether I should just wait it out.

Hi Jimmy,

I have been doing the ketogenic diet, but concerned that I am getting way too many calories. I remember Dr. Thomas Seyfried saying in a podcast that the ketogenic diet is only effective if done in the context of calorie restriction. Otherwise, could result in too much inflammation. Is there something to this?

I've always heard that one should not count calories. My problem however is that, while I can go for long periods of time without eating, when I do eat lunch and dinner, I seem to be a bottomless pit. I add 2 tablespoons of fat to each meal. If I add in more fat in order to reduce my hunger, I'm concerned I will get way too many calories. Last night for example, I hate over 2000 cal before I was satiated, and even then I barely was...

I am 5'3, weigh about 118 pounds and am moderately active. How do I figure out what is the optimal caloric intake for a ketogenic diet?

Hello Jimmy and Doc Muscles,

Thank you so much for the great podcast you have put together. I love the focus on the keto way of life. I listen to all of your other podcasts, Jimmy, and this one is my favorite.

So here is my question... I have heard it hypothesized that you can overeat fat on a keto diet. The theory I have heard is before you can burn your fat stores, you have to burn through the dietary fat you have eaten. Is this for real? I'm trying to understand more about exercise and its effects on ketones. When I go for a good long walk, if my ketones go down, is it because they are being utilized more? I'd like to think that I'm really working on burning the stored fat when I go for a walk versus just the dietary fat I have eaten that day but I'm not sure that is the case.

I also conducted my own N=1 experiment on my blood glucose. I used to come home from work and go for a walk, then come back and have my dinner. I would start out before dinner with a blood glucose of 96-104 then do the post-prandial after and it would go up to about 110-114 before coming down again. Then I decided to eat dinner as soon as I got home and go for a walk immediately after. My blood glucose before dinner was 101. Then I went for a 45 minute walk immediately after dinner and tested again at the one hour mark and it was at 96! Do either one of you have any thoughts about the timing of exercise when it relates to eating and its effects on glucose and ketones?

I've noticed that when I do a strict ketogenic diet and extended fasting, my cortisol shoots through the roof. I have interrupted sleep, wake up at 4:30 or 5:00 am and can't go back to sleep (which was great at first to get things done! But now is just annoying). I'm wired but jittery, irritable and edgy. I want the benefits of the higher ketones, but without the negatives of high cortisol.  Most of what I've read suggests to increase carbs to bring cortisol down. What can I do?

Hey Jimmy,

I want to say that you have made a huge difference in my life. I switched to keto after listening to your podcasts and reading your book. I have dropped 30 pounds in the first month, and have never felt better. Thanks for all the great info you put out there for us. I do have a question though, I have heard that carbs are necessary for testosterone production. I'm 32 this year and want to make sure that a keto diet won't have any deleterious effects on it. Do you have any thoughts on this?

Hi Jimmy,

Thank you so much for all the hard-to-find information you're putting out there for the rest of us.  You're a great resource!  I love all of your podcasts, especially Keto Talk with Jimmy Moore and the Doc.

I am hoping that you will have time and ability to help me get a question answered.  I have been searching all over the net to find some information about whether a LCHF diet should affect liver function tests, and I can't really find what I'm looking for.  I figured that since you seem to be the king of keto blood test results, you would be just the man to answer my question:

I have been eating LCHF for about a year and a half (and thanks to Dr. Nally's protein calculator, I'm recently down to 50g protein and under 20 carbs - usually way under - leaving about 85-90% fat).  I am not over-weight, but when I started LCHF had been at the top of the normal range for myself and just heavier than I was comfortable with.  I have recently been having some digestive issues, so had some blood tests including a liver panel.  One of the liver measures (ALT, I think, but it could have been AST - the enzymes) came back slightly elevated, so my doc has ordered an upper abdominal ultrasound.  I am scared to death that my doc is going to say that I need to change my diet, but I really want to stay keto (even though I really haven't seen the same level of benefits my husband has, I still think it's so good for me).  So here's the question:

I know that some blood tests look different for those on LCHF (such as cholesterol) and that it's not necessarily a bad thing. Is there any reason to think that elevated liver enzymes is normal for someone on LCHF and not a bad thing?  Or might LCHF cause elevated liver enzymes and actually indicated faulty liver function?  Or are these things completely unrelated. 

Thanks for your help!  I have so many keto questions (for someone who is not overweight - which I think is different than the typical situation), but I decided to pick the most pressing one, so I will continue to listen to your podcasts and continue to learn and hopefully get my other questions answered!
