I made the ketogenic lifestyle change over a year ago and my A1c has dropped down from 6.2 to 5.4. I listen to Keto Talk every week and you guys have trained me to look closely at ingredient labels. I eat a lot more heavy cream and my question for you today has to do with the mono and diglycerides as well as the carrageenan that are added to heavy whipping cream. Some people claim these are harmless and others say they are horrible. What I want to know is, do they mess with ketosis? Are they dangerous to your health in any way? I appreciate your feedback on this one.

3. Are there certain non-grass-fed/organic meats that should absolutely be avoided if you are pursuing healthy ketosis?

Hi Jimmy and Dr. Cole,

I've just read Dr. Cate Shanahan's book Deep Nutrition and now I'm baffled as what to eat on a ketogenic diet (from a safe and healthy perspective). I simply can't afford grass-fed beef. I can only get it at a health food store and it's way too expensive for me. I normally shop at Costco and Sobey’s grocery store. I buy pork only from humanely-raised brands. That’s really the only feasible healthy meat choice for me to consume. I buy organic whenever I can. I like beef liver, beef heart, and chicken liver, but they don't ever come from grass fed or even organic sources. What would you advise to NOT eat at all amongst these non-grass-fed/organic choices for meat? What should I be eating on my keto plan that is very healthy that maybe I’m not thinking about? How do I stay on budget in my finances while pursuing optimal health through nutritional ketosis? I've heard some say you need to do the best you can, but I really want to know which foods to avoid if you can't get the good quality.

4. Could the detoxification that takes place from eating keto cause the lymph nodes to swell temporarily until things calm down in the body?

Dear Jimmy and Will,

My question for you guys is about the lymph nodes. Is it possible, since the lymphatic system is a filtering process, that swollen lymph nodes after going keto could be the result of the change in your diet? When you switch over to keto, your body does a natural detox, right? So is it feasible that swollen lymph nodes could be the result of this detoxification and given some time and a few adjustments, such as more water and possibly a lymphatic massage, they could go down on there own without any medical intervention? I understand that it probably depends on how swollen they are along with other factors. I was just curious if my hypothesis could be true?

4. Why did I experience reactive hypoglycemia after eating a ketogenic meal?

Hey Jimmy and Dr. Cole,

I’m a big fan of you guys and four days into eating keto. I feel like I’m starting to get the hang of this and wanted to run something by you. I ate the following food a couple of days ago: one medium avocado, two eggs over easy, two Italian sausages, 3 tablespoons of sour cream, and 3 tablespoons of Kerrygold grass-fed butter. After consuming this food, I almost immediately felt flush, dizzy, and off. I started researching Dr. Google and it looks like I had reactive hypoglycemia. Is this normal on a ketogenic diet or what did I do wrong? Interestingly, I had a fatty coffee and pastrami with cream cheese today and didn’t have any of those same symptoms from he other day. Could it be the extra fat in my meal today compared to the other day? Thanks for the advice on this one.

5. Are there concerns that medicinal herbs and adaptogens can play a negative role in a patient’s health much in the same way as a pharmaceutical drug?

Hey Jimmy and Will,

I listen to you guys all the time and Will I hear you talk about various herbs and adaptogens that you’ve found useful in your clinical practice for your patients basically every week. You also talk about the side effects that come from prescription medications. As a fellow medical practitioner, I see both herbs and pharmaceutical drugs as basically extra chemicals substances that you are adding to a patient’s body in the hopes of eliciting a helpful or healing response. All these extra chemicals and compounds, whether food/plant based OR pharmaceutical based, potentially can interact with each other in very bad ways.

In the hospital where I work, I’ve seen examples of herbs causing liver and bleeding problems and interacting with other medications. The word “natural” does not particularly reassure me; cocaine, toxic mushrooms, poison ivy, and castor beans are all natural and they are also all noxious. A lot of the pharmaceuticals we use (penicillin, digitalis, the list goes on and on) are basically purified natural products. Pharmaceuticals, at least in theory, have to go through a rigorous testing process including ruling out the very powerful placebo effect.

My problem is that herbs and such don’t have to go through that same scrutiny before being made available to the general public. We really have no idea what the dosage of the active ingredient(s) might be from bottle to bottle, brand to brand, of all these herbs, do we? I’ve been low-carb, high-fat, ketogenic for the past five years, was a featured success story in Keto Clarity, have been on Jimmy’s podcast, and teach at an Osteopathic medical school now. I certainly have no objection to herbs, adaptogens, and other natural health supplementation and would never support banning them. But just how well tested are they?

Hey Jimmy and Adam,

Craig and Maria Emmerich say in their book KETO that taking exogenous ketones should be avoided because the body can’t store them. On the oxidative priority list alcohol is number one followed by exogenous ketones, carbs, protein, and then fat. Since alcohol and exogenous ketones can’t be stored in the body, it immediately begins working to get rid of it as a matter of priority. They say that fat comes in last on the list because it is the body’s preferred fuel source. According to them, when these exogenous ketones are ingested, lipolysis (fat burning) stops. At least that’s how I understood the explanation. This is so frustrating for me to try to understand because exogenous ketones are pushed so strongly in the ketogenic community. I want my body burning stored fat, not exogenous ketones. I’m so very confused on this issue.

My question is about fertility. I have heard that a keto diet is very beneficial for woman who are trying to get pregnant. I have a friend who has been trying for a very long time and it seems that she has what some have described as “killer cells.” What does that mean and can a ketogenic diet help? 2. Why do I feel funny in my head and nose side effects when my ketone levels go higher after consuming a Bulletproof coffee?

Hi Jimmy and Doc Muscles,

When my ketones are elevated, I start to get unwanted side effects and feel kinda funky in the head to the point that I have to lie down and I even get a weird sensation in my nose. This morning after putting Brain Octane Fuel from Bulletproof in m coffee with cream, about 30 minutes later my blood glucose was 84 and blood ketones at 3.3. That’s when I get that issue. What is that? I take 45mg NP Thyroid every morning a couple of hours before my coffee or eating, but no other prescriptions.

3. Is it possible to be so well keto-adapted that your body can go in and out and back into ketosis more quickly even after a high-carb meal?

Hey Jimmy & Adam,

Last night, my fiancé and I went out to eat at an Italian restaurant for a friend’s birthday dinner and I decided to cheat. I had a salad, squid ink pasta w/lobster, and for dessert…a big slice of cake. Needless to say, this was a very high-carb, low-moderate protein, and low-fat dinner. I’ve been doing keto on and off for almost 2 years now, and this morning my fasting blood sugar showed 83 and blood ketone was 2.3. I’ve never experienced anything like this before.

Is it possible that I’m just so well keto-adapted now that my body can go in and out and back into ketosis much more quickly? I must also mention that because this high-carb meal was premeditated, I prepped for it by drinking a green drink w/apple cider vinegar and some kimchi before we left for the restaurant. Do you think this helped? I’m also in fairly good shape, and workout quite often so that might have also played a factor. I’ve also heard that eating carbs separately from protein and fats helps with digestion and vice versa.

Do you guys think that the low protein and low-fat helped with my body’s process of allocating the sugar properly so I was able to get right back into ketosis again?

– Do I need to moderate my protein or moderate my fat intake in order to see success at losing weight on a ketogenic diet?

Hi Jimmy & Dr. Nally,

Thank you for doing Keto Talk. I binge listened the last two months and now I’m all caught up. Thanks for the great show because I’ve learned so much. Atkins was my first foray into this community a couple of decades ago and the Grain Brain, Paleo, Dr. Jason Fung and many other followed. Learning more about keto has become my favorite pastime and I only just discovered all these amazing podcasts. There’s one question I haven’t heard anyone provide a good answer for yet and I’m hoping you guys are up for the challenge of answering it. It has to do with the concept of gluconeogenesis. I’ve heard you and Dr. Nally state that it can play a significant role in kicking you out of ketosis and raising blood sugar levels which is why you need to moderate protein intake. You also discuss eating a high-fat diet to satiety and that this within the context of a low-carb nutritional intake does not lead to weight gain. You’ve noted on several occasions that if fat is consumed in excess of what is needed, then the body excretes the extra fat out of the breath, urine, and feces.

However, there seems to be two very opposing views amongst the greater keto world and I’d like to know which side is right and why you think the other side is wrong. One the one side, you’ve got people who say that gluconeogenesis is not something to worry about and that eating a higher protein intake will not affect ketosis (or fat adaptation) and that dietary fat should be moderated for those needing to lose weight so your body can tap into those fat stores instead of using dietary fat for energy. On the other side, you have people like you guys who say the body is burning both stored body fat and the dietary fat that you eat simultaneously so you do not gain weight when you eat more fat. This side also thinks you should moderate down on the protein to prevent the insulin and blood sugar effects that can come from consuming this macronutrient in excess of what the body needs.

So who’s right? Is there some middle ground between these two positions? My main question is whether moderating protein or moderating fat is the best way to shed pounds which is my current goal. It seems that many people listening to this and other leto-focused podcasts are interested in weight loss, so we need to be crystal clear about what will work to make that happen. I understand everybody needs to find the version of keto that works for them but this is difficult for the common person to figure out. Can you help me and others determine how we can figure all this out so we can be successful on keto?

I am a registered dietitian who is an advocate for the keto diet since starting it for myself nine months ago. I am working diligently on influencing others in my profession about the health benefits of nutritional ketosis. Keto Talk has helped me stay on top of what is happening in the world of keto so I can be armed with the knowledge to pass along to my RD colleagues.

Recently I had braces put into my mouth in preparation for jaw surgery coming in the Fall. This will include my mouth being banded together for a few weeks after surgery which would thus limit the types of foods I could eat or drink. Do you have any input about what my recovery will be like being in a state of ketosis vs. if I wasn’t ketogenic? I would assume inflammation would be lower on keto, but is there any research showing how much quicker healing will take place? My wife does not eat keto and has hinted several times that she wants me to just eat whatever post-surgery so I keep my energy up and have an enjoyable experience. I’m not wanting to lose any more weight in this, but the faster recovery would be a bonus.

1. Is it possible to heal my Type 2 diabetes through a ketogenic diet and lifestyle change alone without going back on medications again?

Hello Jimmy and Dr. Cole,

I am a 38-year old male with Type 2 diabetes. I am 6'3" and currently weigh 307 pounds. I started keto at the beginning of the year and have lost 25 pounds so far. I stopped taking all my diabetes medications on am only on blood pressure medication at the moment. My blood glucose numbers run between 155-200 no matter what time of day I check it. I have been implementing intermittent fasting with keto for the past month. So my question for you guys is this: Did I stop taking my medications for controlling Type 2 diabetes too soon? I was taking 1000mg metformin twice a day, 20mg glipizide twice a day, 10 units of lantis per day. I feel so amazing on keto and never want to go back to eating any other way. Do I need to go back on the medications for a short period of time until I heal a little more or will my body heal with a ketogenic diet and lifestyle changes over time?

Most shocking to me when when my doctor told me I’d be on medication for the rest of my life and that diet and lifestyle don’t make a difference with Type 2 diabetes. In fact, one of his nurses told me that I was not insulin resistant. How the heck can a Type 2 diabetic NOT be insulin resistant? The idea of going back on these prescriptions that force me to eat more carbohydrates to counteract their effects is frustrating to me. Can you help me figure this out?

2. Does having an ApoE4 require me to reduce my saturated fat intake to prevent the atherogenic small dense LDL particles from developing?

Hi Jimmy and Will,

I have an ApoE4 that researchers say makes me more susceptible to having higher LDL cholesterol as well as an increased risk for developing heart disease. I eat LCHF and although my LDL-C is high (150) my Small LDL-P count is low at 129 and I’m solidly in Pattern A. My question for you guys is this: Should I be avoiding saturated fats like coconut oil and replacing them with monounsaturated fats like avocados? Will the saturated fats increase the number of small dense LDL particles even though my triglycerides are low?

3. Does the gluconeogenesis that happens from consuming too much protein apply also to the protein found in eggs and plant proteins?

Greetings you guys,

I bought Keto Clarity at Barnes and Noble last week and have read about a third of it so far. When you refer to gluconeogenesis, does that apply to just to meats or is it to eggs and even plant-based proteins as well? Or are the proteins found in eggs and plants different than those in animal-based sources? I’ve always wondered why I never did well on the Atkins diet and this excessive protein issues seems to be the answer I’ve been looking for.

Hi Jimmy and Dr. Cole,

It’s very easy for me to wrap my head around and understand the spike in dopamine that takes place in your brain when you consume sugar. But a goofy thought ran through my head and it made me wonder if dietary fat could have the exact same effect in a fat-adapted person. I know that when I eat fat I always feel really good and happy. So does that mean I had a spike in dopamine? And if so, what are the ramifications for a ketogenic dieter if that is indeed happening?
