Hey Jimmy and Maria,

Of all the tips and answers you've provided to questions around ketogenic diets, I haven't heard anyone talk about the impact they can have on your relationships. I have an amazing wife who is the mother of our two small children. When I started keto diet last year, she was supportive of my efforts for the first six weeks I was doing it. Part of the challenge was I set her expectations that this would only be a temporary thing. However, once I started seeing some incredibly positive results, I had to continue doing it to reap the benefits. But this is when it created a rift in our relationship. She watched me eat meat and cook foods in bacon grease and couldn’t understand how that could possibly be healthy as she subscribes to a more conventional low-fat diet. Additionally, feeding our kids has been hard (although they do love their daddy’s bacon!). So I decided to relax my ketogenic approach in order to appease my wife and introduced carbs back into my diet. Now I want to do periods of intermittent and extended fasting to mitigate the effects of those added carbs. If my wife reacted to keto the way she did, I can’t imagine how she’s gonna respond to the other F-word. My marriage is very important to me and so is my health. Do you have any tips on how a ketogenic spouse can deal with their spouse who is not supporting their ketogenic way of eating? Thanks for all you do!

1. What vitamin and mineral requirements change from what is in the RDA when you adopt a ketogenic diet?

Hey guys,

I've spent a lot of time learning and implementing the exact ketogenic approach that’s right for me. But it occurred to me as I reviewed my micronutrient intake on keto that the RDA levels from our government are based on a carbohydrate-eating population. The sodium recommendation, for example, is 2.3 grams daily which is woefully inept for someone in nutritional ketosis for a wide variety of reasons you’ve already discussed many times on this podcast. But this example of how far off the RDA for sodium has got me thinking what other micronutrients were are way off on with the recommended daily allowance. I read a study that suggests keto dieters need far less Vitamin C than carb-based eaters. So what other vitamins and mineral RDA requirements might be impacted for someone following a well-formulated ketogenic diet? This seems like an interesting subject to explore.

2. Why does a ketogenic diet and fasting lead to a white or yellow coating on the tongue and will it eventually go away?

Many people have pointed out their breath and tongue change while eating a ketogenic diet and during a fast. I’ve seen people posting pictures of their tongue that show a white or yellow coating. What in the world is this and will it eventually go away?

3. Why did my body suddenly stop thriving despite still being in a state of nutritional ketosis?

Hi Jimmy and Maria,

I started my keto diet a couple of weeks ago and it was typical—the first 3 days were miserable, but on day 4 I felt a drastically better as the ketones kicked in. I had five days of incredible energy, suppressed appetite, and superb cognitive benefits as my blood ketones soared into the 2-3 mmol range. It was great! But then things took a change for the worse as I suddenly started feeling cranky, sluggish, and dealing with brain fog. My ketones dipped slightly, but I was still well into ketosis at 1.5+ on the blood meter. I’ve been drinking plenty of water, consuming sodium, magnesium, and potassium for all the electrolyte benefits. So my questions for you guys is this: Did my body start rejecting the ketones I had been thriving on because it wanted to shift back to burning glucose again? Or is this simply a part of becoming keto-adapted? Any suggestions you have about my situation would be greatly appreciated.

– Can a BAC breath alcohol analyzer measure ketone levels accurately? Are ketone levels of 5.0+ dangerous?

Hi Jimmy and Maria,

I have a quick question about using a simple breath alcohol analyzer to test ketone levels. From my understanding, you multiply the number you get on the BAC meter by 4-5 to get the equivalent blood ketone reading. So a 0.4 reading on the BAC would equate to 1.6-2.0 mmol in blood ketones. Is this a valid way to test for ketosis? I know it’s probably inaccurate, but how far off is it? Using this equation to determine my level of ketones with my keto and fasting efforts, I got a 1.3 reading on the BAC which would equate to 5.2-6.5 mmol on the blood ketone meter. Is this a dangerous level of ketones to have?

I listen to your podcasts everyday and I think you guys are awesome speakers. I'm kinda desperate for some answers as I am HIV positive taking HAART (Genvoya) and have lost 13 pounds rather easily eating keto. I love this way of eating, but for some reason while I'm in ketosis I just don’t feel right. I tried it for several weeks and even kept my salt/potassium/magnesium intake up to help balance my electrolytes. Being on a ketogenic diet, I’ve noticed my sleep get deeper after I used to deal with insomnia regularly. During ketosis I slept like a baby...almost too deep of a sleep if that's possible because my head is swimming all day after waking up from this. Is there some metabolic issue with the HIV medications I am taking that could be preventing me from getting the cognitive and energy benefits of ketones? For now, I'm just eating low-carb but avoiding ketosis because of these negative effects.

Any info would be greatly appreciated!

2. Why is my heart racing since I started keto and when can I expect things to return to normal again? Would intense exercise be good or bad during this time?

Hey Jimmy and Will,

I'm a 25-year old guy that started doing keto about a week and a half ago and over the past three days my heart has been racing. I usually have a resting heart rate around 55-60 bpm, but when I started eating keto it jumped up to around 75 bpm. I've been doing a lot of research on this and most of what I'm finding deals with electrolyte imbalances. I have been salting my foods with pink Himalayan sea salt, using lite salt for potassium, and magnesium. How long does it usually take to rebalance all the electrolytes when all of these things are being dumped at the beginning of keto so my heart rate will return to normal? And should I be doing high intensity exercise while my body is still adjusting to keto or would that make this issue worse? I'm starting to get a little worried about it.

3. Can keto help endometriosis? Will food allergies prevent me from going keto? If dairy is estrogenic, should I avoid them if I have high estrogen?

I have several questions for you guys to answer. Is keto effective for people dealing with endometriosis and endometriosis chocolate cysts (endometriomas)? I already know that I have a moderate intolerance (which means I can only have them every 4 days) to eggs whites, cow's milk, whey, coconut and avocado. I know most people just suspect they are intolerant, but I did blood tests that confirm it. Would these food allergies prevent me from going keto? I’ve heard that dairy is inflammatory for the body and can carry extra estrogens, so should women with high estrogen avoid dairy altogether?

– How is it possible for A1c to fall into a healthy range while fasting insulin precipitously rises?

Hi guys,

I am a devoted listener and have found so much information useful to me on my health journey. I am a very active, very trim 5'6" 115-pound woman who is 55 years old living in the Colorado forest. For as long as I can remember, I have been eating a ketogenic diet for 6 months of the year (during the fall and winter) for most of my adult life and have gone fully keto year round the past two years. I drink wine from Dry Farm Wines but no other alcohol. My question is about some blood test results that have me scratching my head about what to make of it. I do regular home A1c tests and I’ve noticed it drop from 5.8 down to 5.0. Perfect response on keto. But when I had my insulin checked over that same time period, I noticed it has risen from 8 to 15 in just six months while eating keto. How is this possible? Any help would be greatly appreciated.

I am such a big fan of Keto Talk and have a quick question for you guys to address. I’m in a Facebook discussion with a calories in, calories out carb-lover. He says there is no such thing keto-adaptation that takes upwards of 4-6 weeks. This fits within his narrative and I would like to rebut that if possible. Do you have any evidence from research that discusses the shift from sugar burner to fat burner and how long it can take? I remember Dr. Nally used to say that cell membrane transporters can take 4-6 weeks to optimize for optimal ketone utilization. Can you help me respond to this keto skeptic?

1. Why have my episodes of vertigo increased since I started eating a ketogenic diet?

Hi Jimmy and Dr. Cole,

I am very grateful for your podcasts as I find them extremely helpful to me and my family as we continue on this ketogenic lifestyle. I am a 33-year old female and was diagnosed with vertigo seven years ago. These episodes were always sporadic when they would hit every couple of years. But I’ve noticed since I started keto that these episodes are happening with greater frequency. Perhaps it coincidence that they started happening more often when I started eating low-carb, high-fat, but I’m wondering if this is something I should expect to be happening more when eating this way. In five months of keto, I’ve lost 55 pounds and feel amazing except for the vertigo issues. Any ideas you can share about this with me?

2. Can keto help me normalize my cortisol levels so I can stop my progressive weight gain?

Hey Jimmy and Will,

I had my metabolism tested with one of those at-home blood and saliva tests and it came back saying I have abnormally low cortisol. I tested myself because I have been rapidly gaining weight even though I follow a low-carb diet and exercise regularly. I’ll admit my low-carb diet up until lately has not been ideal because it is probably too low-fat and too high-protein. I now know better after reading Keto Clarity. Everything I have read about having low cortisol talks about it within the context of having Addison’s disease. I don’t have any other symptoms of Addison’s and I’m wondering why I’m having these low cortisol readings. Would a low-carb, moderate protein, high-fat diet help remedy the cortisol issues? My objective is to be healthy, but I desperately want to stop gaining weight despite eating well. I have always been normal weight my entire life. Any insights you could give me are appreciated.

3. Is there any relationship between histamines and a positive benefit coming from consuming a ketogenic diet?

Have you ever discussed the relationship between histamines and the role a ketogenic can play in helping with this issue? I’ve been having terrible itching and hives and my doctor put me on Zyrtec and Zantac. – What can a Ketonian share with their non-keto primary care physician to encourage him to learn more about a low-carb, high-fat diet?

Hey Jimmy and Dr. Cole,

I'm fortunate enough to live in one of the best cities in the U.S. for health care—Houston, Texas where the Texas Medical Center is located. But even with all of the medical schools, doctors, and health professionals in this city, I have not yet been able to find a primary care physician who has adequate knowledge about keto within a reasonable driving distance of my home. Like most of us Ketonians, I will likely need to do what I can to educate my current primary care physician about the ins and outs of a low-carb, high-fat, keto approach. Do you have any suggestions about how I can begin this process with my doctor in the very limited 10ish minutes I have with him as a patient? What specific information can I convey to my doctor that would make a positive initial impression about keto to encourage him to learn more? What are the key points to underscore and what should I never NOT talk about because it would immediately trigger a negative response? Or is all of this an exercise in futility? Help!

What exactly is the science behind WHY carbohydrates, even the so-called whole food ones, make my joints hurt? And why does eating keto cure my joint pain? 1. Why did my white blood cell count drop to “very low” levels since I went keto?

What can I do to raise it back up again? Hi guys, I love the show and especially love your passion for this topic. I have a question for you to address that is a bit concerning. My wife and I are both 55 years old and in very good health. We went Paleo a couple of years ago and became fully keto last year. My wife had blood work done recently that showed her white blood cell count was very low at 2.9. See below. In looking back at one of her pre-keto labs from 2015, her WBC was low at that time as well at 4.1. But now it has dropped into the "very low" range of 2.9 since going fully keto. Is there any relationship between being in a state of nutritional ketosis and low white blood cell count? Do you have any recommendations on how to boost her WBC? Should I increase my protein?

2. Why did I lose my period when I started eating keto?

Why is my skin still so dry despite eating a ketogenic diet? Dear Jimmy and Will, I thoroughly enjoy listening to Keto Talk. I’ve been eating this way for a few months and really feel great. I have always been sporty and active living at a healthy weight with a good diet. But I have suffered from multiple illnesses over the years, including orthostatic hypotension, hypermobility spectrum disorder, psoriasis, and very dry skin especially on my forearms and shins. Much to my amazement, since adopting a keto lifestyle, my balance has completely resolved after two decades of problems with this! Additionally, the psoriasis appears to be settling down. After listening to so many of your podcasts, I realize my multiple illnesses, 10 years of PTSD, and 8 concussions have caused some major inflammation in my body. So with all this context about me, here are my two concerns: Why have my periods stopped since going keto when I’ve always been regular before? Is this normal hormonal reaction and when should I expect it to restart? And why is my skin still very dry? Will it eventually clear up?

3. Can you explain the relationship between insulin resistance and high calcium levels in easy-to-understand language?

Hi Jimmy and Dr. Cole! I recently figured out that I have hyperparathyroidism (hypothyroid, but hyperPARAthyroid) because I kept having high calcium levels and ending up in the hospital with AFIB. I started searching everything I could about this condition and what could be contributing to it. Now I’m wondering if my continued insulin resistance and need for insulin despite being keto for several years might somehow be related. To my surprise, a search on hyperparathyroidism and insulin resistance brought up some interesting information on this very subject! Can you explain in everyday language what the connection might be between this condition and IR and if there is reason to have hope that this might play a part in my insulin resistance improving more now that I've had the bad gland removed? I'm only a couple of weeks out of surgery but other symptoms I was experiencing have already gotten much better! I'm just really hoping this will be yet another benefit and want to better understand how high calcium levels impact insulin resistance.

Why did my voice change to sound like an old man since I started eating a ketogenic diet? Hey Jimmy and Dr. Cole, You guys can count me as another person who has experienced vocal issues that have popped up since starting on the ketogenic diet. I started eating keto in July and by September I noticed my voice started sounding fuzzy like a very old man. You’ve briefly addressed this issue before stating that a common culprit is coffee consumption, but I don’t drink very much coffee and in fact a lot less of it since I switched to keto. I also don’t eat chocolate or peppermint. Any clue why I sound like the old guy in the Up movie?
